Case report: Hellp syndrome with acute renal failure ( Hellp syndrome )

Abstract

19 yaşında (G1,PO,AO,YO) 28 haftalık gebe, hastaneye baş ağrısı ve dispne yakınmaları ile kabul edildi. Arka arkaya 8 kez eklampsi krizi geçiren ve akut böbrek yetmezliğine girdiği saptanan olgu kliniğe yatırıldı. Magnezyum sülfat ve taze dondurulmuş plazma tedavisi ile birlikte doğumun indüksiyonla yapılma planı uygulamaya konuldu. Dört saatlik indüksiyon uygulamasına rağmen doğum eyleminde herhangi bir ilerleme olmaması üzerine sezaryan uygulanarak 600 gram ağırlığında O apgarlı kız fetus doğurtuldu. Bu arada olguya 2 ünite taze kan ve 3 ünite taze dondurulmuş plazma verildi. Operasyondan 6 saat sonra diürez başladı. Operasyondan 36 saat sonra kan basıncı 150/100 mmHg'a düştü, bilinç geri geldi. Operasyondan üç gün sonrasında trombosit, hemoglobin ve bilirubin düşük düzeylerde kaldı. Operasyondan 9 gün sonra hemoglobin hala düşük düzeyde iken karaciğer enzimleri, trombositler ve böbrek fonksiyon testleri normal düzeylere dönmekteydi. Operasyondan 2 hafta sonra kan basıncı, karaciğer enzimleri ve kan trombositlerinin normal değerlere dönmesiyle olgu hastaneden taburcu edildi.

A 19 - year-old female (Gravida 1. Para O, Abortus O, Live 0) was admitted to the hospital at her 28th week of gestation with headache and dyspnea. After her admission to the clinic she had nine seizure episodes and urine incontinance. Magnesium sulfate and fresh frozen plasma therapy were started. A delivery was planned by induction. After a 4 hours follow-up Caesarean section was performed because of labour arrest. A 600 gr fetus was delivered with 0 Apgar score. Two units of packed fresh blood and 3 uniths of fresh frozen plasma were given. Diuresis was started 6 hours after operation . Blood pressure decreased to 150/100 mmHg, 36 hours after operation she got conscious. On third postoperative day hemoglobin and biluribine levels were decreased while platelets was still low. Liver enzymes, platelets and urinary function tests were normal and hemoglobin was low on 9th postoperative day. She was discharged with normal blood pressure values, blood enzyme profile and platelet counts two weeks after the operation.
