Perkütan nefrolitotomi operasyonlarında ultrasonografi eşliğinde yapılan erektör spina plan bloğunun etkinliği

Abstract

Bu çalışmanın amacı; perkütan nefrolitotomi (PNL) operasyonlarında, preoperatif dönemde ultrasonografi (USG) eşliğinde yapılan erektör spina plan (ESP) bloğunun, intraoperatif opioid ihtiyacı, postoperatif analjezik etkinlik ve hasta memnuniyeti üzerine etkisini gözlemlemektir. Çalışmaya, elektif koşullarda PNL operasyonu planlanan, 18-65 yaş arası, preoperatif değerlendirmesinde ASA (American Society of Anesthesiologists) risk sınıflaması I- II- III olan, 60 hasta dahil edildi. Bütün hastalara bilgilendirilmiş gönüllü olur formu imzalatıldı. Bütün hastalara genel anestezi protokolü uygulandı. Hastalar; preoperatif dönemde ESPB yapılan (blok grubu) ve yapılmayan (kontrol grubu) olmak üzere randomize iki gruba ayrıldı. Çalışma kapsamında elde edilen verilerin istatistiksel olarak karşılaştırılmasında Mann Whitney U testi ve Independent Samples t testi kullanıldı. Gruplar arası karşılaştırmada, demografik veriler açısından istatistiksel olarak anlamlı fark yoktu (p>0,05). Hastaların intraoperatif opioid ihtiyacı, postoperatif 0., 1., 2., 6., 12., 24. saatlerdeki VAS skorları, postoperatif analjezik ihtiyacı ve hasta memnuniyeti açısından gruplar arasında blok grubu lehine anlamlı farklılık olduğu görüldü (p<0,001). Çalışmamızdan elde edilen veriler ışığında; preoperatif dönemde USG eşliğinde yapılan ESPB, PNL operasyonlarında, intraoperatif opioid ihtiyacını ve postoperatif analjezik ihtiyacını azaltmakta ve hasta memnuniyetini artırmaktadır.

The purpose of this study is to observe the effect of ultrasonography (USG) guided erector spinae plane block (ESPB) on intraoperative opioid need, post-operative analgesic efficiency and patient satisfaction during pre-operative period and percutaneous nephrolithotomy (PNL) operations. For this study, 60 patients, aged between 18 and 65, whose risk classification is I-II-III in the preoperative evaluation according to the ASA (American Society of Anaesthesiologists), were included under elective conditions. All patients signed written informed consent. They received general anaesthesia protocol. During the pre-operative period, the patients were randomized into two groups as the block group whom ESPB was applied and the control group whom ESPB wasn't applied. In this study, for the statistical comparison of the obtained data, Whitney U test and Independent Samples t test were used. According to the demographic results there wasn't a statistically significant difference between the groups (p>0,05). According to the patients' intraoperative opioid need, their post-operative VAS scores at 0., 1., 2., 6., 12., 24., hours; their post-operative analgesic needs and patient satisfaction there was a significant difference between the groups in favour of the block group (p<0,001) The results of the study indicate that the ultrasound-guided erector spinae plane block decreases the intraoperative opioid need and post-operative analgesic need. As well as it increases patient satisfaction during pre-operative period and percutaneous nephrolithotomy operations.
