Standart oral glukoz tolerans testi ile izole birinci saat plazma glukozu yüksek olan bireylerde mean platelet volume(MPV), nötrofil lenfosit oranı(NLO) ve platelet lenfosit oranı(PLO)

Abstract

Standart oral glukoz tolerans testi ile izole birinci saat plazma glukozu yüksek olan bireylerde mean platelet volume (MPV), nötrofil lenfosit oranı (NLO) ve platelet lenfosit oranı (PLO), Kırıkkale Üniversitesi Tıp Fakültesi İç Hastalıkları Anabilim Dalı, Uzmanlık Tezi, Kırıkkale, 2019. Amaç: Ortalama trombosit hacmi (MPV), platelet lenfosit oranı ve nötrofil lenfosit oranı gibi kronik inflamasyon belirteçlerinin 75 gramlık standart oral glukoz tolerans testi (OGTT) ile izole 1.saat plazma glukozu 155mg/dl ve üstünde olan bireylerdeki düzeylerini tespit ederek prediyabetin ve diyabetin erken tanısı için yeni belirteçler saptamak. Materyal Metot: 1 Ocak 2016 ile 31 Aralık 2017 tarihleri arasında standart OGTT yapılmış ve OGTT'de izole 1.saat yüksekliği saptanan 50 hasta ve normal glukoz toleransı saptanan 48 birey kontrol grubu olarak çalışmaya dahil edilmiştir. GruplarMPV, NLO ve PLO parametreleri açısından birbiriyle karşılaştırılmıştır. Bulgular: İki grup arasında yaş, cinsiyet ve VKİ açısından anlamlı fark yoktu(p>0,05). Hasta grubunda 1.saat, 2.saat plazma glukoz değerleri, açlık insülin, HOMA-IR değeri ve beyaz küre sayısı kontrol grubuna göre anlamlı düzeyde yüksek bulundu (p<0,05). Hasta grubunda PLO anlamlı düzeyde düşük bulundu (p<0,05).İki grup arasında lipid parametreleri, HbA1c, CRP, MPV, NLOdeğerleri açısından anlamlı fark yoktu (p>0,05). Sonuç: Elde ettiğimiz sonuçlara göre standart OGTT sırasında izole 1. saat yüksekliği bulunan hastaların normal glukoz toleransı olarak kabul edilmesinin doğru olmayabileceğini ve bu konuda daha fazla hastanın daha uzun süre sonuçlarının takip edildiği prospektif dizaynda ileri çalışmalara gereksinim olduğunu düşünüyoruz. Mean platelet volume (MPV), neutrophil lymphocyte ratio (NLO) and platelet lymphocyte ratio (PLO) in persons with only elevated 1-hour postload glucose level during standard oral glucose tolerance test, Department of Internal Medicine Kırıkkale University Medicine Faculty, Specialist Thesis, Kırıkkale, 2019. Objective: We investigated the levels of chronic inflammation markers such as mean platelet volume (MPV), platelet lymphocyte ratio and neutrophil lymphocyte ratio in patients with 1st hour plasma glucose levels of ? 155 mg / dl during 75 mg standard oral glucose tolerance test (OGTT). In this way, we aimed to identify new markers for early diagnosis of prediabetes and diabetes. Material and Methods: Fifty subjects with only elevated 1st hour postload glucose as the patient group and 48 subjects with normal glucose tolerance as the control group were included in the study between 1 January2016 and 31 December2017. Groups were compared in terms of MPV, NLO and PLO parameters. Results: There were no significant difference between the two groups for age, gender and BMI (p> 0.05). In the patient group, 1st and 2nd hour postload glucose levels, fasting insulin, HOMA-IR and white blood cell count were significantly higher than the control group (p <0.05). PLO was significantly lower in the patient group (p <0.05). There were no significant difference between two groups in terms of lipid parameters, HbA1c, CRP, MPV and NLO values (p> 0.05). Conclusion: According to the our results, we think that it may not be accurate to describe persons with only elevated 1st hour postload glucose as a normal glucose tolerance. Further studies are needed in the prospective design which more patients are followed up for longer periods of time. Key words: OGTT, Glucose, MPV, NLO, PLO
