Septum deviasyonunda nazal hava yolunun değerlendirilmesinde klasik muayene yöntemleri, akustik rinometri ve nazal spirometrinin preoperatif ve postoperatif olarak karşılaştırılması

Abstract

Amaç: Bu çalışmada , septoplasti yapılan hastalarda nazal hava yolu anterior rinoskopi , nazal endoskopi, akustik rinometri (AR), nazal spirometri ( S), oral spirometri (OS) ve vizüel analog skala (V AS) ile değerlendirildi ve bu yöntemlerin etkinliklerini birbirleri ile karş ı laştı rı ldı. Gereç ve Yöntem: çalışmamıza, izole septum devi asyon u tanı sı ile sadece septoplasti ameli yatı yap ıl an 27 hasta dahil edildi . Kontrol grubu olarak 30 sağ lıklı bireyalındı. Bu ha talara preoperatif anterior rinoskopi, nazal endoskopi, nazal spirometri, oral spirometri, akustik rinometri ve VAS yap ıldı. Hastalar postoperatif 6-8 hafta sonra tekrar aynı test batarya l arı ve muayene yönteml eri ile değerl e ndirildi . Bulgular: Preoperatif ve postoperatif V AS değe rle ri karş ılaştırıldığında tüm parametrelerle te t onalama değerleri aras ınd a istatistiksel olarak yüksek oranda önemli fark elde edildi (P<0,05). Her iki nazal kavite preoperatif ve postope ratif AR parametreleri karşıl aştırıldı ğında test onalama değerleri aras ında sadece solda voll ve msefl 'de istatistiksel olarak an l amlı fark tespit ed ildi . Preoperatif ve postoperatif NS ve OS parametreleri kendi içlerinde karş ılaştırıldığınd a istatistiksel olarak önemli fark saptanmad ı (P>0,05 ). Ancak, istati stiksel olarak anlamlı olmasa da tüm OS parametrelerinde postoperatif artış gözlendi. NS ve OS parametreleri nin büyük oranda birbirleriyle uyum içinde o ldu ğu bulundu. Sonuç: VAS her ne kadar subjektif bir yöntem olsa da postoperatif hasta d eğer l endirilmes ind e önemlidir. AR sadece burnun amerior kı s mının değerl endirilmes inde yararlı bir yöntem iken, septoplasti endikasyonu koymada ve ameliyat başarısını değerl e ndirmede yardımcı bir yöntem oldu ğu kuşku lu bulunmuştur. NS'nin septum deviasyonu ya da septoplasti başarı sının objektif olarak değerl endirilmesi n e katkıda bulunmadığı fikrine varılmı ş tır . OS septoplasti yapılacak h astal arın ameliyattan sistemik anlamda ne kadar faydal andığını gösterebilen, kolay uygu lanabilir, objektif bir yöntem olarak değerlendirilmişıir. Methods and Materials: Twenty seven patients whom underwent only septoplasty for septum deviations were included in this studyand 30 healthy ubjects were included as control group. The patients were evaluated preoperati vely with anterior rhinoscopy, nasal endoscopy, NS, OS, AR, and VAS. All patients were evaluated with the same test batteries and examination method s 6-8 weeks af ter the operation. Results: When we compared the preoperative and postoperative V AS scores to each other, we found tati tically important difference beıween the mean test values of all parameıers (pO,OS). Although there was not a statistical1y significant differences, it was detected increase in all postoperative OS parametres. We found that the values of S and OS parameters are in good relationship with each other. Conclutions: Even though V AS is a subj ectiye method, it is imponant for evaluating patients after surgery. While AR is a good method in evaluating anterior nasal area, it is found suspicious in making septoplasty indication and evalııating the success rate of the operation. We are in the opinion that NS is not usefull in eva iuating objecti vely the sepıum deviation or the success rate of septopl asty. OS is found as an objecti ve, and easy method to use and can show the postoperative benefit rate from the septoplasty operation.
