    Amantadinin alt ekstremite iskemi/reperfüzyon sıçan modelinde akciğer dokusu üzerindeki etkileri

    Amaç: Bu çalışmada, sıçanlarda alt ekstremite iskemi/reperfüzyon\rhasarından sonra amantadinin akciğer dokusu üzerindeki etkisi araştırıldı.\rÇalışma\rplanı:\rToplam 24 adet Wistar cinsi sıçan her grupta altı\rsıçan olacak şekilde dört eşit gruba ayrıldı: Sham grubu (Grup S),\ramantadin grubu (Grup A), iskemi/reperfüzyon grubu (Grup I/R) ve\rI/R+amantadin grubu (Grup I/R-A). Tüm gruplara orta abdominal insizyon\ruygulandı. Grup I/R ve I/R-A?da infrarenal abdominal aorta 120 dk.\rsüreyle klemplendi ve ardından klemp kaldırılarak 120 dk. süreyle\rreperfüze edildi. Grup A ve I/R-A?daki sıçanlara 45 mg/kg amantadin\rhidroklorür cerrahiden 15 dk. önce intraperitoneal olarak uygulandı.\rReperfüzyon periyodu sonunda (240 dk.) tüm sıçanlar sakrifiye edildi ve\rakciğer dokuları alındı. Akciğer dokusunda katalaz ve süperoksit dismutaz\raktiviteleri ve glutatyon S-transferaz ve malondialdehit düzeyleri çalışıldı.\rAkciğer dokuları histopatolojik olarak incelendi.\rBulgular:\rKatalaz aktivitesi Grup A, I/R ve I/R-A?da, Grup S’ye kıyasla\rdaha düşüktü. Süperoksit dismutaz aktivitesi Grup I/R?de, Grup S?ye kıyasla\rdaha yüksekti. Grup I/R-A ve A?da, Grup S ve I/R?ye kıyasla, süperoksit\rdismutaz aktivitesi azaldı. Glutatyon S-transferaz düzeyleri Grup I/R ve\rA?da, Grup S’ye kıyasla azaldı. Glutatyon S-transferaz düzeyleri, Grup I/RA’da,\rGrup I/R ve A’ya kıyasla daha yüksekti. Malondialdehit düzeyleri,\rGrup I/R’de en yüksek ve Grup IR-A?da en düşük izlendi. Histopatolojik\rincelemeye göre, Grup S?de infiltrasyon skoru Grup I/R ve Grup I/R-A?ya\rkıyasla, anlamlı düzeyde daha düşüktü (sırasıyla. p=0.009 ve p=0.011).\rGrup I/R?de alveol duvar kalınlaşması skoru, Grup S ve Grup A?ya kıyasla,\ranlamlı düzeyde yüksekti (sırasıyla, p=0.001 ve p=0.001).\rSonuç:\rAkciğer dokusu iskemi/reperfüzyon hasarından etkilenebilir ve bu\rhasar amantadin kullanımı ile geri döndürülebilir.

    Comparison of the efficacy of propofol and metoclopramide in preventing postoperative nausea and vomiting after middle ear surgery

    Objectives: To compare the administration of sub hypnotic dose of propofol with metoclopramide and placebo in prevention of postoperative nausea and vomiting (PONV) after middle ear surgery. Methods: This clinical research was performed in the Faculty of Medicine, Gazi University, Besevler, Ankara, Turkey, between December 2004 and October 2005. Following approval by the hospital ethics committee, 60 adult patients scheduled for a middle ear operation were randomly assigned into 3 groups. The patients in group P received 0.5 mg.kg(-1) propofol; in group M, 0.2 mg.kg(-1) metoclopramide, and in group C, 0.9% saline solution. The number of patients suffering from nausea and vomiting at 0-4, 4-12, and 12-24 hours postoperatively, and additional use of antiemetics was recorded. Results: Comparison of the data showed that at 0-4th hours, the incidence of vomiting was 25% in group P, 40% in group M, and 75% in group C. The incidence rate of group P was significantly lower than that of group C (p=0.002), and the rate of antiemetics use in group C was higher than that in group P (p=0.028). The Nausea Vomiting Scale scores of group C were also significantly higher than those of group P (P=0.005). There were no significant differences between the values at 4-12 and 12-24 hours. Conclusion: The administration of a sub hypnotic dose of propofol at the end of surgery was found to be at least as effective as metoclopramide in preventing PONV in the early postoperative period in adult patients undergoing middle ear surgery.

    The Effects of HES 130/0.4 Application on Erythrocyte Deformability in Ureteral Obstructed Rats

    Amaç: Plazma genişleticilerinin uygulanması, önemli cerrahi, hemorajik şok ve travmayı içeren çeşitli klinik koşullardaki kritik hastaları yönetmek için çok önemlidir. Bununla birlikte, hidroksietil nişastanın (HES) eritrosit agregasyonu üzerindeki etkileri tartışmalıdır. Bu nedenle, deneysel tek taraflı üreteral tıkanıklık ile oluşturulan böbrek yetmezliğinde HES 130/0.4'ün eritrosit deformabilitesi üzerindeki etkilerini araştırmayı amaçladık. Yöntem: Toplam 24 rat kontrol, HES 130/0.4, Üreter Obstrüksiyon (ÜO) ve ÜOHES 130/0.4 olmak üzere dört gruba ayrıldı. Üreteral tıkanıklık gruplarına ketamin anestezisi altında düşük abdominal insizyon yapıldı. Sağ üreterin distaline ulaşıldıktan sonra 2.0 mersilen ile bağlandı, daha sonra geç dönemde böbrek yetmezliği için 3 hafta bekledi. 20 mL.kg-1 HES 130/0.4 (Voluven), HES 130/0.4 ve ÜO-HES 130/0.4 grubuna intravenöz olarak verildi. 24 saat sonra ratlar sakrifiye edildi. Deformabilite ölçümleri bir fosfat tamponlu salin (PBS) tamponu içinde% 5 hematokrit kullanılarak gerçekleştirildi. Bulgular: Her grupta rölatif rezistans kontrol grubuna göre arttı (p<0.0001). Her grupta eritrosit deformabilite indeksi kontrol grubuna göre anlamlı derecede yüksek bulundu (Grup K-Grup HES, p=0.023, Grup K-Grup ÜO, p<0.0001, Grup K-Grup ÜO-HES, p<0.0001) . ÜO-HES grubunda HES grubuna kıyasla eritrosit deformabilite indeksi önemli derecede arttı (p=0.031, p=0.021, sırasıyla). HES 130/0.4 uygulaması ile üreteral obstrüksiyon yapılmış sıçanlarda eritrosit deformabilitesinde herhangi bir değişiklik olmadığı belirlendi (p = 0.785). Sonuç: HES 130/0.4 kullanımı, üreteral obstrüksiyonu yapılan sıçanlarda eritrosit deformabilitesi üzerinde olumsuz bir etkiye sahip değildir. HES 130/0.4 kullanırken bu endikasyonların doğru kullanılması gerektiğini düşünüyoruz

    The influence of sevoflurane anesthesia on the rat red blood cell deformability

    Alterations in blood rheology under the influence of anesthesia have been observed and discussed among the responsible factors for the deterioration of tissue and organ perfusion related to anesthetic procedures. Sevoflurane is one of the volatil anesthetics which is being used very common in surgery. In this study, the effects of sevoflurane anesthesia were investigated in different age groups of rats. 22 male rats were used in the study and the rats were divided into two groups according to their age (young and old) comprising of two subgroups in each. First group was the young control (n = 5), the second was the young group treated with sevoflurane (n = 5), the third group was the old control (n = 7) and the last group was the old group treated with sevoflurane (n = 5). %2 of sevoflurane was applied to the rats with inhalation in a adjustable cage for one hour. The deformability indexes of the erythrocytes were measured by a laser diffractometer (Myrenne Rheodyne SSD). Deformability indexes of red blood cells were significantly decreased with sevoflurane in old rats (p = 0.028) whereas it had not any significant effect in young group compared with their controls. When we compared the young and old control groups, the deformability indexes were significantly higher in old ones (p < 0.001). However, there were not any significant difference between the old and the young sevoflurane applied groups. A volatil anesthetic agent sevoflurane has impaired the deformability of erythrocytes in old rats compared to their controls, whereas it had not any significant effect in young ones which may be due to the flexibility of the young erythrocytes leading them to tolerate to the environmental changes. These results reveal that the inhalation anesthetics like sevoflurane may cause more serious problems in the elder people and their hemodynamic parameters should be checked more seriously during the surgery. © 2006 - IOS Press and the authors.     The red blood cell deformability alterations under desfluran anesthesia in rats

    General anesthesia, either with inhalation or through nonvolatile anesthetics, is known to affect the overall cardiovascular function as well as the microcirculatory hemodynamics. In this study, the effects of desfluran anesthesia on the red blood cell deformability of young and old rats are investigated. 33 male rats were used in the study and the rats were divided into two groups according to their age (young and old) comprising of two subgroups in each. First group was the young control (n=5), the second was the young group treated with desfluran (n=7), the third group was the old control (n=7) and the last group was the old group treated with desfluran (n=7). %6 of desfluran was applied to the rats with inhalation in an adjustable cage for one hour. The elongation indexes of the erythrocytes were measured by a laser diffractometer (Myrenne Rheodyne SSD). Deformability indexes of red blood cells were significantly increased with desfluran in young rats (p=0.042) whereas they were significantly decreased in old rats (p=0.004) with desfluran application compared with their controls. When we compared the young and old control groups, the deformability indexes were significantly higher in old ones (p < 0.001). However, there was no significant difference between the old and the young desfluran applied groups. The volatile anesthetic agent desfluran impairs the deformability of erythrocytes in old rats compared to their controls, whereas it has the opposite effects on young ones. This may be due to the alterations in membrane structure with age. These results reveal that the inhalation of anesthetics like desfluran may cause more serious problems in the elder people during the surgery and may influence their hemodynamic parameters.

    The effects of amantadine on lung tissue in lower limb ischemia/reperfusion injury model in rats

    Background: This study aims to evaluate the effect of amantadine on lung tissue of after lower limb ischemia/reperfusion injury in rats. Methods: A total of 24 Wistar rats were divided into four equal groups including six rats in each: sham group (Group S), amantadine group (Group A), ischemia/reperfusion group (Group I/R), and ischemia/reperfusion + amantadine group (Group I/R-A). All groups underwent a midline abdominal incision. In Groups I/R and I/R-A, the infrarenal abdominal aorta was clamped for 120 min and, then, reperfused for 120 min after removal of the clamp. Amantadine hydrochloride 45 mg/kg was administered intraperitoneally to the rats of Groups A and Group I/R-A 15 min before surgery. At the end of reperfusion period (240 min), all rats were sacrificed, and their lung tissues were obtained. Lung tissue catalase and superoxide dismutase activities and glutathione S-transferase and malondialdehyde levels were analyzed. Lung tissues were examined histopathologically. Results: Catalase activity was lower in Groups A, I/R, and I/R-A compared to Group S. Superoxide dismutase activity was higher in Group I/R than Group S. Superoxide dismutase activity in Groups I/R-A and A decreased, compared to Groups S and I/R. Glutathione S-transferase levels decreased in Groups I/R and A, compared to Group S. Glutathione S-transferase levels in Group I/R-A were higher than Groups I/R and A. The highest level of malondialdehyde was found in Group I/R and the lowest level was found in Group I/R-A. According to histopathological examination, infiltration scores were significantly lower in Group S than Groups I/R and I/R-A (p=0.009 and p=0.011, respectively). The alveolar wall thickening scores in Group I/R were also significantly higher than Groups S and Group A (p=0.001 and p=0.001, respectively). Conclusion: Lung tissue can be affected histopathologically by ischemia/ reperfusion injury and this injury can be reversed by amantadine administration.

    The Effects of Esmolol on Erythrocyte Deformability in Rat Liver Ischemia-Reperfusion Injury

    Background: Esmolol has protective effects in ischemia reperfusion (IR) injury. The purpose of our study was to look into the effects of this which esmolol on erythrocyte deformability in rat liver IR injury model. Materials and Methods: We used 24 Wistar albino rats as subjects in our study. They were divided into 4 groups; randomized control group (group C; n=6), esmolol group 200 mu g/kg/min intravenously (group E; n=6), IR group (group IR; n=6) and IR group with esmolol 200 mu g/kg/min intravenously (group IR-E; n=6). Erythrocyte packs were prepared from heparinized blood samples and deformability measurements were performed. Results: It was discovered that ischemia reperfusion increased the relative resistance when compared to control group (p<0.0001). Erythrocyte deformibility index was found to be higher in IR and IR-E groups compared to control group (p<0.0001, p=0.002, respectively). Esmolol application decreased the erythrocyte deformibiltiy index when compared to control group (p=0.017). Conclusion: In this research, esmolol application has improved the ertyhrocyte deformibity in liver rat IR injury partially. We also found that esmolol had beneficial effects by reversing undesirable effects of IR. Further studies with larger volume are required to support our promising results.
