    The efficiency evaluation of the training activity given to ambulance personnel

    Aim: It was aimed to evaluate the efficiency of the three days module given to the assistant health personnel working in 112 ambulances. Methods: It was assumed that all the personnel working in the 112 ambulance systems under the health directory of Kırıkkale city would be involved. Theoretic and practical first aid training was given to all the personnel attending to the course in 6 groups of 20-25 with duration of 3 days for each. A 50 itemed questionnaire was applied to all the groups before and after the training. Results: 132 people working in ambulance system in Kırıkkale city were involved in training. 63.6% of the personnel involved in the study were male and 66% were married. When they were grouped according to age, it was determined that 44% of them were in 26-35 age group. When they were grouped according to branches, it was determined that 47% of them were emergency medical technician (EMT) and 40.2% were health officer. When their level of education was taken into consideration, it was determined that 53% of them were graduates of high school and 43.2% were graduates of associate degree. 50.8% of all the personnel had a job experience of 1-5 years. When all the groups were evaluated together, there was a statistical meaningful difference (p<0.001) between pretest success grade (29.58±6.74) and posttest success grade (39.81±4.68). When EMT, paramedics and the other groups were compared; there was a pr test grade difference(p=0,003) between EMT and paramedics group(31,55±6,52) and the others' group(27.70±6.4). There wasn't a grade difference between EMT, paramedics group and the others' group in posttest (p=0.765). Conclusion: The occupational training module which was done in a limited time was determined to be effective in providing a meaningful positive difference in knowledge level and providing personnel knowledge level standardization.

    Injuries related to the feast of holy sacrifice

    Objective: The aim of this study is to evaluate and draw attention to sacrification injuries that occurred during the Feast of Holy Sacrifice. Methods: Two hundred and sixty-two patients who were admitted to the emergency services in the city of Kirikkale with sacrification injuries during four consecutive Feast of Holy Sacrifice holidays between 2008 and 2011 were evaluated according to age, sex, admission day and time, experience level, type and mechanism of injury and treatment type. Results: Average age was 43 ± 14 and 83 % of were male. Only 7 patients (2.7 %) were professional butchers, while 155 patients had sacrificed previously. Eighty-three patients had not sacrificed previously and 24 patients had only witnessed sacrification. Penetrating injuries were 81 % (206 cases from knives and 6 cases from animals' horns) and 19 % were blunt. The admittance to the EDs was highest on the first day. Upper extremity trauma constituted for 80 % of cases (n = 209) and 197 (94 %) were hand injuries, whereas 39 patients (15 %) had lower extremity injury. While 174 patients (67 %) injuries were self-inflicted with a knife, 32 (12 %) were injured by the knife of another and 50 (21 %) were injured by animal aggression. Primary suture (155 patients), splint and tendon repairing were undertaken. No deaths occurred. Conclusions: Sacrification injuries can sometimes be serious with the injuries consisting mainly of tendon cuts and bone fractures. Preliminary arrangements with medical staff and equipment support should be provided during the Feast of Holy Sacrifice, especially on the first day. The butchering jobs during this period should be given to professionals. © 2013 Springer-Verlag Berlin Heidelberg.

    Isotretinoin intoxication in attempted suicide: A case report

    We report a case of acute intoxication due to a massive overdose of isotretinoin. A 17 year-old male patient had ingested 16 capsules of isotretinoin (20 mg) with suicidal intentions. He presented with nausea, dizziness and myalgia on extremities. 2 hours after the attempt he was brought to our clinic. We administered gastric lavage and active charcoal treatment before taking him to the intensive care unit for observation. 24 hours later, cutaneous xerosis and desquamation of the face especially the nasolabial region occurred; cutaneous xerosis resolved spontaneously. The side-effects were only mild exacerbations of some common isotretinoin side-effects. There was a low toxicity of isotretinoin overdose. To date, few cases of isotretinoin overdosages have been reported. Being alert when using isotretinoin on a teenager may save life because this drug may exacerbate depression as a side effect.
