Öğe Can Dehydroepiandrosterone-Sulphate be a New Diagnostic Parameter in Idiopathic Hypogonadotropic Hypogonadism?(Galenos Publ House, 2022) Çadırcı, Kenan; Keskin, Havva; Bayrak, Muharrem; Çarlıoğlu, Ayşe; Ceylan Arıkan, SenayObjective: Dehydroepiandrosterone (DHEA) and its sulfate derivative DHEA-sulfate (DHEA-s) are major androgen hormones which are synthesis from the adrenal origin. The purpose of this study was to investigate DHEA-s levels in male patients with idiopathic hypogonadotropic hypogonadism (IHH) and to determine whether DHEA-s level are a useful marker for diagnosis of IHH.Methods: A total of 91 subjects, 31 males with IHH (mean age 19.7 +/- 2.6 years) and 60 healthy males (mean age 20.7 +/- 2.6 years), were enrolled in this study. The patients with IHH were selected from the subjects who had not yet started treatment for hypogonadism and who had no additional disease, while the healthy control group consisted entirely from individuals admitted to the same hospital outpatient clinic for routine check-ups. Both groups' blood sampling, anthropometric measures, and physical examination were undertakenResults: Mean DHEA-s level was 133.4 +/- 56.5 mu g/dL in the IHH group and 433.3 +/- 160.3 mu g/dL in the control group (p=0.000). The low DHEA-s level in patients with IHH was independent of age, cortisol, and adrenocorticotropic hormone (ACTH) at multivariate logistic regression analysis. The ROC analysis showed that DHEA-s <= 38.2 mu g/dL supports a diagnosis of IHH with 100% specificity and 100% sensitivity. DHEA-s was as predictive as total testosterone which is used in the diagnosis of patients with IHH.Conclusion: DHEA-s level was significantly lower in the males with IHH compared to controls. Therefore, DHEA-s may be a potential predictive marker for diagnosis of IHH.Öğe Evaluation of Serum Lipid Parameters in Acromegaly Patients(Yerkure Tanitim & Yayincilik Hizmetleri A S, 2019) Keskin, Havva; Cadirci, Kenan; Kaya, Yasemin; Durmaz, Senay; Arslan, Abdulmuttalip; Yildiz, Faruk; Carlioglu, AyseObjective: In acromegaly, the incidence of cardiovascular disease is higher compared to healthy population because of the association of secondary cardiovascular disease risk factors such as hypertension, glucose intolerance, dyslipidemia, and insulin resistance. This study aimed to evaluate the relationship between Growth Hormone (GH), Insulin-like Growth Factor-1 (IGF-1), and lipid profiles in patients with newly diagnosed acromegaly. Methods: This study included 57 newly diagnosed acromegaly patients and 31 healthy control cases. Blood samples were taken after a 12-hour night fasting to analyze the levels of the hormones, the lipid profiles, and the fasting blood glucose. Results: The mean of LDL-C level in the acromegaly group and control group were respectively 140.2 +/- 61.3 mg/dL and 115.6 +/- 30.4 mg/dL, (p=0.029), triglyceride level 137.6 +/- 84.0 mg/dL and 102.9 +/- 43.0 mg/dL, (p=0.027) and HDL-C level 41.5 +/- 10.5 mg/dL and 50.5 +/- 10.6 mg/dL, (p=0.001). There was a significant positive correlation between GH levels of LDL (r=0.375, p=0.002), TG (r=0.302, p=0.01), IGF-1 (r=0.634, p=0.0001) and HOMA-IR (r=0.699, p=0.0001), IGF-1 and LDL levels (r=0.295, p=0.01) and TG (r=0.476, p=0.0001). There was a negative correlation between HDL and GH (r=-0.399, p=0.001) and HDL with IGF-1 (r=-0.310, p=0.01). Conclusions: Acromegaly is a disorder characterized by an excess of GH and GH appears to adversely affect the lipid profile. It can be considered that this risk factor may be eliminated by the control of the levels of GH and IGF-1 in acromegaly patients. Further studies seem necessary for more clarifica-tion.Öğe Investigation of the neutrophil/lymphocyte and monocyte to high-density lipoprotein cholesterol ratios in differentiated thyroid cancers(2020) Dal, Derya; Yıldız, Faruk; Keskin, Havva; Bayrak, Muharrem; Özmen, Hilal Kızıltunç; Durmaz, Şenay; Çarlıoğlu, AyşeIntroduction: Thyroid cancers are the most common malignant tumors of endocrine origin. They are classified depending on their histopathological and clinical behaviors. Papillary and follicular cancers are classified as differentiated thyroid carcinomas (DTCs). The neutrophil/lymphocyte ratio (NLR) and the monocyte to highdensity lipoprotein cholesterol ratio (MHR) have recently been shown to be powerful markers of oxidative stress and systemic inflammation, and the MHR has been revealed as a potent marker of mortality in coronary heart disease associated with coronary atherosclerosis. The aim of this study was to evaluate these markers in patients diagnosed with DTC. Material and Method: One hundred twenty-five patients newly diagnosed with DTC and a 75-member control group consisting of entirely healthy individuals were included in the study. The patient and control groups were evaluated by investigation of cholesterol and hematological parameters following 12-h fasting. Results: Significant differences were determined between the patient and control groups in terms of mean NLR (3.2±2.8 vs 2.4±1.3, respectively, p=0.013) and MHR (0.102±0.079 vs, 0.038±0.052 respectively, p<0.001) values. In the correlation analysis, positive correlation was determined between the NLR and white cell count (r=0.530, p<0.001), neutrophil count (r=0.293, p<0.001) and C reactive protein (CRP) (r=0.371, p=0.005), while negative correlation was determined between the NLR and lymphocyte count (r=-0.271, p=0.001). Conclusion: The study data show that DTCs increase systemic inflammation.Öğe MONOCYTE TO HDL-CHOLESTEROL RATIO IN MALE WITH HYPOGONADOTROPIC HYPOGONADISM(2019) Çadırcı, Kenan; Oğuz, İsra; Usta, Tuba; Carlıoğlu, Ayşe; Keskin, Havva; Arıkan, ŞenayINTRODUCTION: Previous studies have shown that idiopathic hypogonadotropic hypogonadism (IHH) patients with low testosterone levels lead to an impaired glucose metabolism and an increased cardiovascular risk. Monocyte to high-density lipoprotein cholesterol (HDL-C) ratio (MHR) also has been shown to be an indicator of inflammation and cardiovascular risk. The purpose of the present study is to investigate MHR in the male with IHH. MATERIAL AND METHODS: This study includes 31 men with IHH without previous treatment for the disease and 44 healthy men. The blood samples, anthropometric measures, and physical examination were undertaken by all the participants. RESULTS: Mean ages of the IHH patients and the healthy controls were 22.5±7.2 years vs. 22.9±6.4 years. There was no statistically significant difference between the patients and controls in terms of the mean HDL-C, mean monocyte count, and mean MHR. The MHR also was not correlated with the other hematological parameters, inflammatory parameters, and the total testosterone level. CONCLUSION: MHR has been shown to be an indicator of cardiovascular risk in previous studies but we could not detect an increased MHR value in patients with IHH in this study.Öğe Polikistik over sendromlu hastalarda artmış ürik asit düzeyleri ve klinik ile ilişkisi(2016) Keskin, Havva; Timur, Özge; Kaya, Yasemin; Utlu, Mustafa; Yıldız, Faruk; Dilekçi, Esra Nur Ademoğlu; Kamalak, ZeynepAmaç: Polikistikover sendromu (PKOS) üreme çağındaki kadınları etkileyen en sık endokrin bozukluktur. Klinik ve biyokimyasal özellikleri metabolik sendrom ile benzerlik göstermektedir. Metabolik sendromlu hastalarda insülin direnci, serum ürik asit düzeyi ile ilişkilidir. Çalışmamızda PKOS hastalarında serum ürik asit düzeylerinin laboratuar ve klinik ile ilişkisini incelemeyi amaçladık. Gereç ve Yöntemler: Hastanemiz Endokrin Kliniğin'de takip edilen 172 PKOS hastası, 34 yaş ve vücut kitle indeksi (VKİ) uyumlu kontrol grubu toplam 206 olgu katılmıştır. Erken folliküler fazda, 12 saat açlık sonrası kan örnekleri alınmıştır. Bazal serum hormon düzeyleri immunoassay ile değerlendirildi.Bulgular: Ürik asid düzeylerinin PKOS grubunda kontrol grubuna göre istatistiksel olarak anlamlı bir şekilde daha yüksek saptanmıştır (4,25 1,07 PKOS, 3,7 0,79 kontrol, P 0,048). Ürik asit ile VKİ, total kolesterol, insülin, siklus süresi, Ferriman Gallwey skoru arasında pozitif anlamlı korelasyon, HDL kolesterol ile anlamlı negatif korelasyon bulunmuştur. Çok değişkenli logistik regresyon yapıldığında ürik asit düzeylerinin PKOS grubunda yaş ve VKİ'den bağımsız olarak iki kat daha yüksek olduğu saptanmıştır.Sonuçlar: Çalışmamızda PKOS grubunda ürik asit düzeyleri anlamlı bir şekilde yüksek bulunmuş ve bu yüksekliğin yaş ve VKİ'den bağımsız olduğu görülmüştür. Ürik asit ile kardiyovasküler risk faktörü olan kolesterol, Trigliserit, VKİ, insülin arasında pozitif korelasyon görülmüştür. Klinik gösterge olan Ferriman Gallwey, siklus süresi açısından da pozitif korelasyon saptanmıştır. Artmış ürik asit düzeyleri PKOS hastalarında kardiyovasküler hastalık oluşumu için bağımsız bir risk faktörü olabilir, klinikte olumsuz sonuçları gösterebilir
