    Facial pain: sinus or not?

    Facial pain remains a diagnostic and therapeutic challenge for both clinicians and patients. In clinical practice, patients suffering from facial pain generally undergo multiple repeated consultations with different specialists and receive various treatments, including surgery. Many patients, as well as their primary care physicians, mistakenly attribute their pain as being due to rhinosinusitis when this is not the case. It is important to exclude non-sinus-related causes of facial pain before considering sinus surgery to avoid inappropriate treatment. Unfortunately, a significant proportion of patients have persistent facial pain after endoscopic sinus surgery (ESS) due to erroneous considerations on aetiology of facial pain by physicians. It should be taken into account that neurological and sinus diseases may share overlapping symptoms, but they frequently co-exist as comorbidities. The aim of this review was to clarify the diagnostic criteria of facial pain in order to improve discrimination between sinogenic and non-sinogenic facial pain and provide some clinical and diagnostic criteria that may help clinicians in addressing differential diagnosis.

    Impact of obesity on infertility in women

    The prevalence of obesity and overweight are increasing and have become an epidemic worldwide. Obesity has detrimental influences on all systems, including reproductive health. The prevalence of obesity in infertile women is high, and it is well known that there is an association between obesity and infertility. The relationship between obesity and reproductive functions is still being explored. Overweight women have a higher incidence of menstrual dysfunction and anovulation. Overweight and obese women are at a high risk for reproductive health. The risk of subfecundity and infertility, conception rates, miscarriage rates, and pregnancy complications are increased in these women. They have poor reproductive outcomes in natural as well as assisted conception. These poor reproductive outcomes include assisted reproduction such as ovulation induction, in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI), and ovum donation cycles. Weight loss has beneficial effects on the reproductive outcomes in these patients.

    Current trends and progress in clinical applications of oocyte cryopreservation

    Purpose of review To delineate the current trends in the clinical application of oocyte cryopreservation. Recent findings Although the first live birth from oocyte cryopreservation was reported approximately three decades ago, significant improvement in the clinical application of oocyte cryopreservation took place only over the past decade. On the basis of the available evidence suggesting that success rates with donor oocyte vitrification are similar to that of IVF with fresh donor oocytes, the American Society of Reproductive Medicine has recently stated that oocyte cryopreservation should no longer be considered experimental for medical indications, outlying elective oocyte cryopreservation. Meanwhile, a few surveys on the attitudes toward oocyte cryopreservation revealed that elective use for the postponement of fertility is currently the most common indication for oocyte cryopreservation. Most recently, a randomized controlled trial revealed important evidence on the safety of nondonor oocyte cryopreservation, and confirmed that the clinical success of vitrification is comparable to that of IVF with fresh oocytes. Summary The evidence suggesting similar IVF success rates with both donor and nondonor cryopreserved oocytes compared with fresh oocytes will increase the utilization of elective oocyte cryopreservation. Appropriate counseling of women for oocyte cryopreservation requires the establishment of age-based clinical success rates with cryopreserved oocytes for various indications.

    Use of Doppler ultrasound as prediction of preeclampsia at the early pregnancy

    Doopler ultrasound may be useful for prediction of preeclampsia by performing with the first or second trimester ultrasound pregnancy screening. This can be increase the patient adjusment and also decrease the cost of the screening. According to the first trimester studies, sensitivity of Doopler ultrasound is lower than second trimester studies. But first trimester Doppler ultrasound give a chance of early prediction and prophylaxis for preeclampsia. However sensitivity and PPV values of first trimester studies must be increase. By the way unuseless drug consumption will be prevented at the normal pregnant population. For increase the sensitivity and PPV values, to profit by serum markers may be helpful. However for routine screening of preeclampsia, Doppler ultasound combination with the serum markers is still limited. For this reason wide longitudinal population studies are neccessary for the use Doopler ultrasound and serum markers as a screening test. There is no cost effectiveness studies about Doopler ultrasound and serum marker combinations which are necessary to perform.

    Thyroid physiology and pregnancy

    Thyroid disease is very frequent in women, during pregnancy and the postpartum period. The healthy pregnant women is considered to be euthyroid. However, the production, circulation, and disposal of thyroid hormone are altered in pregnancy. The pattern of thyroid hormones in pregnancy is important in diagnosis thyroid disease. The main events occurring during pregnancy are: a marked increase in serum thyroxinebinding globulin levels, a marginal decrease in free hormone concentrations, a direct stimulation of the maternal thyroid gland by elevated levels of human chorionic gonadotropin (hCG), and modifications of the peripheral metabolism of maternal thyroid hormones. The placenta transports and metabolizes maternal thyroid hormones.

    Submandibüler sialolitiazis zemininde Sphingomonas paucimobilis enfeksiyonu

    Sphingomonas paucimobilis (S. paucimobilis) gram negatif, aerobik, non-fermentatif, oksidaz () ve katalaz () bir bakteridir. S. paucimobilis, nadir şekilde izole edilmesine rağmen, nozokomiyal ve toplumdan kazanılmış enfeksiyonlara neden olabilir. Kliniğimizdeki bir hastanın bir haftadır sağ submandibüler bölgede şişlik ve yemek yerken artan ağrı yakınması vardı. İki elle yapılan muayenesinde sağ Wharton kanalında 1x1x1 cm boyutunda ağrılı şişlik belirlendi. Sağ submandibüler beze yapılan masaj ile Wharton kanal ağzından gelen pürülan akıntıdan kültür alındı ve S. paucimobilis üretildi. Ultrasonografide kanalda ya da bezde taş saptanmadı, fakat tedavi sırasında submandibüler beze masaj yapılarak sağ Wharton kanalından iki adet taş geldiği gözlendi. On dört günlük intravenöz ampisilin-sulbaktam tedavisi ile enfeksiyonun eradike olduğu gözlendi. Hastanın altı ay sonraki kontrol muayenesinde herhangi bir patolojik bulgu ya da belirti saptanmadı ve ultrasonografisi normal idi. Bilgilerimize göre bu hasta sialolitiazis zemininde S. paucimobilis üretilen ilk siyaladenit olgusudur.
