    Can Thiol/Disulphide Homeostasis Help in The Differential Diagnosis of Appendicitis in Children?

    Objective: We aimed to investigate the potential of assessing thiol/disulfide homeostasis as novel oxidative stressmarkers to improve the challenging diagnosis of acute appendicitis in children.Material and Methods: A total of 60 pediatric patients (0-18 years) were enrolled in the study, 30 of which were inthe control group and 30 in the acute appendicitis group. Native thiol, total thiol, dynamic disulfide, dynamic thiol,ischemia modified albümin, albumin, White blood cell, hemoglobin, hematocrit, and platelet counts of both groups weremeasured. The results of both groups were compared using the SPSS (Statistical Package for Social Sciences) version17 (Chicago, USA) program. For all variables, p <0.05 was considered significant.Results: Total thiol (p<0.001), native thiol (p<0.001), and albümin (p<0.001) levels were significantly decreased whiledynamic disulfide, dynamic thiol (p=0.003), and ischemia modified albümin (p<0.001) levels those indicating oxidantside were increased in acute appendicitis group compared to control. White blood cell counts in acute appendicitisgroup were higher (p<0.001), and platelet counts were lower (p=0.03) than the control group.Conclusion: In the differential diagnosis of acute appendicitis, especially in case that are difficult to diagnose, besides aphysical examination, imaging, and current laboratory tests, quantification of thiol/disulfide homeostasis may be helpfulin diagnosing. In addition, evaluating albumin and IMA levels may increase the specificity of the test. This test can bemore helpful in cases diagnosis is difficult such as children small in their ages and mental retardation.

    Does Subclinical Hypothyroidism Affect Dynamic Thiol/Disulfide Homeostasis and ischemia-modified Albumin Levels in Children?

    Objective: To determine the effects of subclinical hypothyroidism on oxidative stress in children. Study Design: A cross-sectional study. Place and Duration of Study: Department of Paediatrics, Paediatric Endocrinology, and General Outpatient Clinics, Kirikkale University, School of Medicine, from May 2017 to October 2018. Methodology: This study included 92 subjects aged between 2 and 18 years. The subjects were divided into two groups. Forty-seven children with subclinical hypothyroidism and 45 healthy controls were evaluated. In order to evaluate oxidative damage, native thiol, total thiol, disulfides, their ratios, and ischemia-modified albumin (IMA) levels were compared between the two groups. The relationship between TSH and IMA levels was assessed. Results: Age and gender were not significantly different in the two groups. Native thiol, total thiol, disulfides and their ratios were similar in the two groups. lschemia-modified albumin levels were significantly higher in the patient group than the controls (p<0.001). There was no correlation between TSH and IMA levels in the patient group (r=0.069 p=0.645). Conclusion: Subclinical hypothyroidism may be related to the impairment of IMA, and have a neutral effect on thiol/disulfide balance. Further research is needed to explain the effects of oxidative stress in subclinical hypothyroidism.

    Evaluation of the effect of chiropractic manipulative treatment on oxidative stress in sacroiliac joint dysfunction

    Objectives: This study aims to investigate the effect of chiropractic manipulative treatment on sacroiliac joint dysfunction (SIJD) and its relationship to oxidative stress (OXS) parameters. Patients and methods: Thirty-three patients diagnosed with SIJD (20 males, 13 females; mean age 36.3 +/- 9.7 years; range, 18 to 60 years) and 30 healthy volunteers (20 males, 10 females; mean age 36.4 +/- 12.2 years; range, 20 to 57 years) were included in this cross-sectional, case-control study conducted between February 2017 and September 2017. Manipulation was applied to the patients once a week for a duration of four weeks. The patients were evaluated at pre-treatment and one month after treatment with visual analog scale, SIJD test, and total thiol, native thiol, disulphide, and ischemia-modified albumin (IMA) as OXS indicators. Results: Prior to treatment, we demonstrated that serum native thiol (mu mol/L) and total thiol (mu mol/L) levels in the patient group were lower compared to control subjects (p=0.03 and p=0.02, respectively). Serum IMA levels were higher in the patient group (p=0.01). There was no change in OXS parameters after manipulative treatment in the patient group. Conclusion: Manipulation is useful in SIJD. Thiol/disulphide homeostasis and serum IMA levels may be used to measure the OXS in patients with SIJD.

    Investigation of Thiol/Disulfide Balance in Obese Rats with Non-Alcoholic Fatty Liver Disease

    Purpose: Due to the increasing prevalence of obesity worldwide, non-alcoholic fatty liver disease (NAFLD) has reached epidemic dimensions over time. NAFLD is the most common cause of childhood chronic liver disease. There is a relationship between NAFLD and oxidative stress. This study aims to investigate the changes in thiol/disulfide homeostasis parameters to determine the oxidant/antioxidant balance in obese rats with diet-induced NAFLD and healthy rats. Methods: Twelve Wistar albino rats were used in this study. Experimentally produced NAFLD obese rats (n=6) and healthy rats were compared. Experimental NAFLD model was created with a special fatty liver diet (Altromin (R) C1063, Fatty Liver Diet, Exclusivet, Lage, Germany). The biochemical and histopathological features of the groups, as well as serum thiol/disulfide homeostasis parameters, were analyzed and compared. Results: In the experimentally induced NAFLD rat model, they gained more weight than the control group. Steatosis (at least grade 2) occurred in all rats fed with special fatty liver diet for 12 weeks. Histopathologically, no high-grade inflammation was observed in rats with experimental NAFLD after feeding a diet for 12 weeks. Results revealed that aspartate transaminase and alanine transaminase levels were high, albumin levels were low, oxidant stress parameters increased, and antioxidant thiol groups decreased. Conclusion: Experimental NAFLD is characterized by increased oxidant stress accompanying fatty tissue in the liver. Analysis of thiol/disulfide homeostasis parameters in NAFLD can be used in further studies to develop effective treatment options.

    KOLESİSTEKTOMİ PLANLANAN HASTALARDA KOLELİTİYAZİSİN VE KOLESİSTEKTOMİNİN TİYOL/DİSÜLFİD DENGESİ'NE ETKİSİ

    Amaç: Bu prospektif kontrollu klinik çalışmanın amacıyetişkinlerde komplike olmamış kolelitiyazisin ve buna yönelikuygulanan laparoskopik kolesistektomi ameliyatının hastalardatiyol/disülfid dengesine etkilerini araştırmaktır.Gereç ve Yöntem: Çalışmaya 18-80 yaş arası sağlıklı gönüllükatılımcılar (Kontrol grubu, n=30) ve 18-80 yaş arasındasemptomatik safra taşı tanısı konulup laparoskopikkolesistektomi planlanan hastalar (KLS grubu, n=30) alındı.Çalışmada sağlıklı gönüllülerden bir defaya mahsus ve tümhastalardan ameliyattan 1 saat önce ve ameliyattan 24 saat sonraiki kez olmak üzere 2 mL venöz kan örnekleri alındı veserumlarda tiyol/disülfid düzeyleri incelendi.Bulgular: Çalışmanın sonunda Kontrol grubunun Nativ Tiyol,Total Tiyol ve Disülfid değerleri KLS grubunun ameliyat öncesiölçülen değerlerine göre daha yüksek bulundu (p<0.001). KLSgrubuna ait ameliyat sonrası bakılan Nativ Tiyol, Total Tiyol veDisülfid değerlerinin Kontrol grubuna göre düşük olduğu tespitedildi (p<0.001). Ayrıca Kontrol grubunda Disülfid/Nativ Tiyolve Disülfid/Total Tiyol oranları KLS grubu ameliyat sonrasıdeğerlerine kıyasla daha yüksek ve Nativ Tiyol/Total Tiyol oranıdaha düşük olarak ölçüldü. Diğer yandan KLS grubununameliyat öncesi ve ameliyat sonrası laboratuvar değerleriarasında da farlılık olmadığı görüldü.Sonuç: Bu sonuçlarla kolelitiyazis hasta grubunda hem ameliyatöncesi hem de ameliyat sonrası doğal tiyol, toplam tiyol, disülfidseviyelerinin düşük olması kolelitiyazis kaynaklı oksidatif stresnedeniyle bu bileşiklerin bu hastalarda tüketildiğini düşündürdü.Diğer yandan bu hastalarda oksidatif metabolizmada olan budeğişikliklerin safra kesesinde taş oluşumunu provakeedebileceği de düşünüldü.

    Significance of thiol/disulphide homeostasis and ischemia modified albumin levels in chronic obstructive pulmonary disease

    Objectives: The severity of inflammation occurring during chronic obstructive pulmonary disease (COPD) isclosely associated with oxidative stress. The aim of this study was to investigate the diagnostic value ofThiol/disulphide homeostasis (TDH) and ischemia modified albumin (IMA) levels in evaluating oxidativestress in COPD patients.Methods: This prospective study was performed with COPD patients presenting to the Kırıkkale UniversityHospital and with healthy volunteers. Subjects’ demographic data (age, sex, body mass index, and smokingstatus), native thiol (NT), total thiol (TT), disulphide (Ds), IMA levels and Ds/NT, Ds/TT and NT/TT ratioswere recorded. Statistical analysis was performed with SPSS 21.0 software.Results: One hundred ninety subjects were enrolled in the study, 141 COPD patients and 49 healthy volunteers.No difference was determined between the patient and control groups in terms of age, sex or body mass index.The antioxidant markers; NT and TT levels and NT/TT ratio were significantly lower in the patient groupcompared to the control group (p < 0.001, p < 0.001, and p < 0.003, respectively). The oxidant markers; IMAlevels and Ds/NT and Ds/TT ratios were significantly higher in the patient group (p = 0.006, p = 0.003, and p= 0.003, respectively). Significant negative correlation was determined between antioxidant and oxidantparameters. Sensitivity values were NT: 87.2%, TT: 83.3%, Ds/NT ratio: 68.1%, Ds/TT ratio: 68.1%, and IMA:77.8%.Conclusions: TDH was impaired in favor oxidants in COPD patients. TDH parameters and IMA can be usedto monitor oxidative stress emerging in COPD.
