Septum deviasyonunun mladina sınıflamasına göre değerlendirilmesi ve akustik rinometri ölçümleri, koku duyusu ve sf-36 ile değerlendirilen hayat kalitesi üzerindeki etkileri

dc.description.abstractBu çalışmanın amacı septum deviasyonu olan hastaların Mladina sınıflamasına göre sınıflandırılması, akustik rinometri ölçümleri, koku fonksiyonlarındaki değişim ve bu durumun hayat kalitesi üzerindeki etkilerini incelemektir. Çalışmaya 18-75 yaşları arasındaki septum deviasyonu olan 20 hasta ve 20 septumu düz birey dâhil edildi. Çalışmaya alınan tüm hastalara KBB muayenelerinin ardından, akustik rinometri ölçümleri, SF-36 yaşam kalitesi ölçeği ve Kısa koku tanıma testi (Brief Smell Identification test) uygulandı. Septum deviasyonu olan hastaların deviasyon tipi; anterior rinoskopik muayene, nazal endoskopik muayene ve daha önceden başka sebeplerle çekilen paranazal BT ile Mladina sınıflamasına göre sınıflandırıldı. Çalışmamızda en sık deviasyon tipi Mladina tip 3 olarak belirlenmiştir. Cinsiyete göre toplam koku skorları açısından septum düz grubunda kadınların bilateral toplam koku skoru değerleri, erkeklere göre anlamlı derecede düşüktür (p<0.05). Toplam koku skoru değerleri ise septum deviye grubu bireylerde kontrol grubu bireylere göre anlamlı derecede daha düşük bulunmuştur (p<0.05). Tek tek koku değerlerine bakıldığında ise tarçın, muz ve sabun kokularının septum deviye grupta septum düz gruba göre anlamlı derecede düşük olduğu görülmüştür(p<0.05). Sonuç olarak septum deviasyonu olan hastalarda olfaktör fonksiyonlarda bozulma olmaktadır. Bu nedenle septum deviasyonunun tedavisi nazal obstrüksiyonun giderilmesinin yanında koku bozukluğunun tedavisi için de gereklidir. dc.description.abstractThe aim of present study is to evaluate the patients with septum deviation according to the classification of Mladina, acoustic rhinometry measurements, changes in smell function and the effect on quality of life.Twenty patients with septum deviation between 18-75 years of age and 20 subjects without septum deviation were included in the study. After ENT examinations, acoustic rhinometry measurements, the SF-36 quality of life scale and the Brief Smell Identification Tests wereapplied to all patients. Type of septum deviation is classified according to Mladina classification by anterior rhinoscopy examination, nasal endoscopy and paranasal CT which was taken previously for other reasons. The study indicated that most common deviation type is Mladina Type 3. Bilateral total smell score of the women in the septum straightgroup is significantly lower than the men in terms of total smell scores by gender (p<0.05). The total smell score values in deviated septum group were significantly lower than those of the control group (p<0.05). According to the individual smell values, cinnamon, banana and soap odors of the septum deviation group were seen to be significantly lower than the septum straight group (p<0.05). The results of the study indicated that, olfactory dysfunction occurs in patients with septum deviation.Therefore, treatment of septum deviation is necessary for improvement of nasal obstruction besides the treatment of the smell dysfunction.
