Yaşlılarda Serum Klotho Protein Düzeyleri İle Kas Gücü Ve Fonksiyonları Arasındaki İlişki Giriş ve Amaç: Sarkopeni ileri yaşla birlikte ortaya çıkan iskelet kası gücü kaybı ile birlikte, kas kütlesi ve fonksiyonunun azalması olarak tanımlanmıştır ve malnutrisyon ilişkili geriatrik sendromlardan biridir. Alfa Klotho protein başta böbrekte olmak üzere tüm dokularda reseptörü bulunan FGF-23'ün ko-reseptörüdür. Fare deneylerinde alfa Klotho protein ekspresyon düzeyinin azalmasının erken yaşlanma ile ilişkili olduğu gösterilmiştir. Bu çalışmada geriatrik popülasyonda sarkopeni ile serum alfa Klotho protein düzeyi ilişkisinin saptanması amaçlanmıştır. Gereç ve Yöntem: Çalışmada 65 yaş üstü, EWSGOP2 tanı kriterlerine göre sarkopenisi bulunan 30 hasta vaka grubuna, sarkopenisi bulunmayan 30 hasta kontrol grubuna dahil edildi. Katılımcıların antropometrik ölçümleri, yürüme hızları, el kavrama güçleri, BİA ile vücut analizleri değerlendirildi. Serum alfa Klotho protein düzeyleri solid faz sandeviç ELİSA tekniği ile ölçüldü. Bulgular: Çalışmamızda vaka grubunda kontrol grubuna göre kas kütlesi ölçümleri, vücut yağ yüzdesi, yağsız kuru ağırlık oranı, total vücut kuru ağırlık oranı, hücresel yağ kütlesi, el kavrama gücü ve yürüme hızı istatistiksel açıdan anlamlı derecede düşük bulundu. Vaka grubunda kontrol grubuna göre alfa Klotho protein düzeyi istatistiksel olarak anlamlı derecede düşük bulundu (p=0,02). Alfa Klotho protein ile kronik hastalıklar, handgrip ve yürüme hızı arasında istatistiksel bir ilişki bulunmadı. Tartışma ve Sonuç: Sarkopeni neden olduğu komorbidite ve mortalite nedeniyle önemli bir geriatrik sendromdur. Alfa Klotho protein düzeyi sarkopenik hastalarda anlamlı derecede düşük bulunmuştur. Bu çalışma sarkopeninin tanı ve tedavisinde alfa Klotho proteinin öneminin anlaşılması için gelecekte yapılacak çalışmalara yol göstermektedir. The Relationship Between Serum Klotho Protein Levels and Muscle Strength and Functions in the Elderly Introduction and Objective: Sarcopenia is defined as the loss of skeletal muscle strength and reduction in mass and function of muscle associated with age. Sarcopenia is one of the geriatric syndromes associated with malnutrition. Alpha-Klotho protein is the co-receptor of FGF-23, which has receptors in all tissues, especially in kidneys. Mouse experiments have shown that decreased expression of alpha-Klotho protein is associated with premature aging. The aim of this study is to detect the relationship between sarcopenia and serum alpha-kotho protein levels in geriatric population. Materials and Methods: In this study, 30 patients aged 65 years and older with sarcopenia according to EWSGOP2 diagnostic criteria were included in the study group and similar aged 30 patients without sarcopenia were included in the control group. Anthropometric measurements, walking speed, handgrip strength, body analysis with BIA were evaluated. Serum alpha-klotho protein levels were measured with solid phase sandwich ELISA technique. Results: In our study, it was found that muscle mass, body fat percentage, lean dry weight percentage, total body dry weight percentage, cellular fat mass, handgrip strength, and walking speed were statistically significantly lower in the study group than in the control group. It was found that alpha-kotho protein level was statistically significantly lower in the study group compared to the control group (p=0.02). There was no significant correlation between alpha-Klotho protein and chronic diseases, handgrip, and walking speed. Discussion and Conclusion: Sarcopenia is an important geriatric syndrome because it leads to comorbidity and mortality. Alpha-klotho protein levels were found to be significantly lower in sarcopenic patients. This study allows of as a guide for future studies to understand the importance of alpha-Klotho protein in the diagnosis and treatment of sarcopenia.
