Septum deviasyonu olan hastalarda nazolakrimal kanalın bilgisayarlı tomografi ile değerlendirilmesi

Özet
Bu çalışmada septum deviasyonu olan hastalarda nazolakrimal kanal proksimalde ve distalde incelenerek septum deviasyonunun nazolakrimal kanal (NLK) çapı ile ilişkisi ve maksiller sinüs (MS) çapıyla ilişkisinin ortaya konması amaçlanmıştır. Septum deviasyonu nedeniyle septoplasti operasyonu yapılan 50 erkek, 50 kadın yetişkin hastanın preoperatif paranazal sinüs bilgisayarlı tomografi (PNSBT) görüntüleri incelenerek, deviye tarafla (ipsilateral) karşı taraf (kontralateral) NLK çapları proksimal ve distalde ölçülmüştür. Ayrıca bilateral MS yükseklik, ön-arka ve horizontal çapları da ölçülmüştür. İpsilateral taraf NLK çapıyla, kontralateral taraf NLK çapı arasında proksimalde veya distalde anlamlı fark bulunmamıştır(p>0,05). Yapılan ölçümler sonucunda NLK'nın deviasyon tarafından etkilenmeksizin proksimalde distalden daha geniş olduğu izlenmiştir(p<0.05). Aynı şekilde deviasyon tarafından etkilenmeksizin NLK'nın erkeklerde kadınlardan daha geniş olduğu izlenmiştir(p<0,05). Çalışmamızda ipsilateral ve kontralateral MS boyutları arasında istatistiksel olarak anlamlı bir fark izlenmemiştir(p>0.05). Sonuç olarak; NLK'nın erkeklerde, kadınlardan daha geniş olması ve proksimalde distalden daha geniş olması; Primer Edinsel Nazolakrimal Kanal Obstrüksiyonunun (PENKO) kadınlarda daha sık görülmesini ve sıklıkla kanalın distalinde obstrüksiyonun görülmesini açıklayabilir. Septum deviasyonunun PENKO' ya neden olan etiyolojik faktörlerden birisi olup olmadığı tıp dünyasında halen tartışmalı bir konudur ve bu konuyu aydınlatma adına ileride yapılacak çalışmalar önem arz etmektedir.
In this study, the nasolacrimal canal was examined proximal and distally in patients with septum deviation to investigate the relationship between septum deviation and nasolacrimal canal (NLC) diameter and to reveal its relationship with maxillary sinüs (MS) diameter. Preoperative paranasal sinus computed tomography (PNSCT) images of 50 male and 50 female adult patients who underwent septoplasty due to septal deviation were examined, and the deviated side (ipsilateral) and opposite side (contralateral) NLC diameters were measured at the proximal and distal parts. More over bilateral MS height, anteroposterior and horizontal diameters were also measured. There were no significant differences between the NLC diameter of the ipsilateral and contralateral sides, either proximally or distally (p>0.05). As a result of the measurements, it was observed that the NLC was wider in the proximal part than in the distal part without being affected by the deviation side (p<0.05). Likewise, it was observed that the NLC was wider in males than females without being affected by the deviation side(p<0.05). In our study, no statistically significant differences were observed between ipsilateral and contralateral MS dimensions (p>0.05). As a conclusion; the NLC is wider in men than in women; and wider proximally than distally. It may explain the fact that Primary Acquired Nasolacrimal Canal Obstruction (PANCO) is more common in women and that obstruction is frequently seen in the distal part of the canal. Whether septum deviation is one of the etiological factors causing PANCO is still a controversial issue in the medical world, and future studies are important to clarify this issue.
