Kırık iyileşmesi sürecinde kandaki preptin hormon seviyesi ve immünhistokimyasal olarak insan kemik dokusunda araştırılması

Özet
Kırık iyileşme sürecinde yer alan mekanizmalar ve olaylar ortopedi ve travmatoloji biliminde büyük ilgi gören bir konu olmaya devam etmektedir. Kırık iyileşmesini etkileyen faktörler ve iyileşmeyi hızlandıran araştırmalar popüler araştırma konularıdır. Özellikle kırık iyileşmesini hızlandırma konusunda birçok farklı çalışma yapılmıştır. Çalışmaya Kırıkkale Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Kliniğine başvuran ve kesin tanısı "uzun kemik kırığı" olarak doğrulanan hastalar ile hasta grubu ile yaş ve cinsiyet bakımından uyumlu herhangi bir tıbbi problemi olmayan sağlıklı gönüllüler dahil edildi. Çalışmaya 25 uzun kemik kırığı olan ve 25 herhangi bir tıbbi problemi olmayan sağlıklı gönüllüler dahil edildi. Bunun yanı sıra preptin hormonunun immünhistokimyasal olarak insan kemik dokusunda gösterilmesi amacıyla femur proksimal uç kırıkları ve koksartroz nedeniyle kliniğimizde opere edilecek ve normal şartlarda atılacak olan femur başları alındı. Femur başlarının immünhistokimyasal boyaması gerçekleştirildi. Çalışmanın sonucunda hasta grubunda sağlıklı bireylere göre preptin değerlerinin hastaneye başvuruları sırasında, ameliyat sonrası birinci günde ve ameliyat sonrası altmışıncı günde yüksek olduğu saptandı. Ayrıca hasta grubunda ameliyat sonrası altmışıncı günde ölçülen ALP değerlerinin de sağlıklı bireylere göre yüksek olduğu bulundu. Diğer yandan parathormon değerlerinin hasta grubunda hastaneye başvuruları sırasında, ameliyat sonrası birinci günde, ameliyat sonrası on beşinci, altmışıncı ve yüz yirminci günde yüksek olduğu tespit edildi. Ek olarak femur başlarına yapılan immünhistokimyasal boyamada özellikle osteoblastlarda preptin hormon immünpozitifliği gösterildi.
The mechanisms and processes involved in the fracture healing process continue to be a topic of great interest in the science of orthopedics and traumatology. Factors affecting fracture healing and studies that accelerate healing are popular research topics. In particular, many different studies have been conducted on accelerating fracture healing. Patients who applied to Kırıkkale University Faculty of Medicine Orthopedics and Traumatology Clinic and whose final diagnosis was confirmed as "long bone fracture" and healthy volunteers who did not have any medical problems compatible with the patient group in terms of age and gender were included in the study. The study included 25 healthy volunteers with long bone fractures and 25 without any medical problems. In addition, to demonstrate the preptin hormone immunohistochemically in human bone tissue, femoral heads, which would be operated on in our clinic due to femoral proximal end fractures and coxarthrosis and would be discarded under normal conditions, were taken. Immunohistochemical staining of the femoral heads was performed. As a result of the study, it was found that preptin values were higher in the patient group compared to healthy individuals during hospital admission, on the first postoperative day and on the sixtieth postoperative day. In addition, ALP values were found to be higher in the patient group on the sixtieth postoperative day compared to healthy individuals. On the other hand, parathormone values were found to be elevated in the patient group at the time of hospital admission, on the first, fifteenth, sixtieth and one hundred and twentieth postoperative days. In addition, immunohistochemical staining of femoral heads showed preptin hormone immunopositivity, especially in osteoblasts.
