Yaman SH. Seröz Otit Tanısıyla Cerrahi Uygulanan Pediatrik Hastalarda Vestibüler Sistemin Değerlendirilmesi, Kırıkkale Üniversitesi Tıp Fakültesi Kulak Burun Boğaz Anabilim Dalı, Uzmanlık Tezi, Kırıkkale, 2024 Bu çalışmada seröz otitis media (SOM) tanısı alıp cerrahi olarak tedavi edilen çocuk hastalarda vestibüler sistemin değerlendirilerek, SOM' un vestibüler sisteme etkilerinin ve cerrahi tedavinin etkinliğinin ortaya konması amaçlanmıştır. Bu prospektif çalışmaya SOM nedeniyle cerrahi uygulanan 40 çocuk hasta (Grup 1) ve ek hastalığı olmayan 20 sağlıklı çocuk (Grup 2) dahil edilmiştir. Grup 1 hastaları cerrahi öncesi, cerrahi sonrası 2. gün, cerrahi sonrası 1. ve 2. hafta video head impulse test (vHIT) ile değerlendirilmiştir. Grup 2 hastaları ise timpanogram ve vHIT ile değerlendirilmiştir. Preoperatif timpanogram ve vHIT sonuçları kontrol grubu ile karşılaştırılmıştır. Veriler karşılaştırıldığında grup 1 hastalarının preoperatif vHIT değerleri ile grup 2 hastalarının vHIT değerleri arasında anlamlı fark bulunamamıştır (p>0,05). Grup 1 hastalarının cerrahi öncesi ve sonrası vHIT sonuçları karşılaştırıldığında belli kanallardaki VOR kazanç ortalamalarında anlamlı farklar saptanmıştır (p<0,05). Yine Grup 1 hastalarının cerrahi öncesi ve sonrası tetkikleri karşılaştırıldığında cerrahi öncesi var olan covert sakkadların cerrahi sonrası dönemlerde azaldığı ve 2. hafta sonunda tüm covert sakkadların kaybolduğu saptanmıştır (p<0,05). Sonuç olarak; SOM, gelişen medikal ve cerrahi tedavi yöntemlerine rağmen hala çocuk yaş grubunda gerek maddi gerek manevi yönden hastaları, aileleri ve sağlık politikalarını etkileyen önemli bir hastalıktır. Özellikle çocuk hastalarda önemli bir dengesizlik nedenidir. Bundan dolayı çocukların ve ailelerin yaşam kalitelerine, çocukların eğitim hayatına olumsuz şekilde etki etmesinin önüne geçilmesi için erken tanı ve uygun endikasyonlar ile medikal ve cerrahi tedavilerinin yapılması gerekir. Çalışmamız sonuçları göz önüne alındığında cerrahi tedavinin vestibüler sistem üzerine olumlu etkisinin olabileceği düşünüldü.

Evaluation of the Vestibular System in Pediatric Patients Who Underwent Surgery with the Diagnosis of Otitis Media with Effusion, Kırıkkale University Faculty of Medicine, Department of Otorhinolaryngology, Specialization Thesis, Kırıkkale, 2024 In this study, it's aimed to reveal the effects of otitis media with effusion (OEM) on the vestibular system and the effectiveness of surgical treatment in pediatric patients diagnosed with OEM by evaluating the vestibular system. This prospective study included 40 pediatric patients who underwent surgery due to OEM (Group 1) and 20 healthy children without comorbidities (Group 2). Group 1 patients were evaluated with video head impulse test (vHIT) before surgery, on the 2nd day after surgery, and at the 1st and 2nd weeks after surgery. Group 2 patients were evaluated with tympanogram and vHIT. Preoperative tympanogram and vHIT results were compared with the control group. When the data were compared, no significant difference was found between the preoperative vHIT values of group 1 patients and the vHIT values of group 2 patients (p>0,05). When the vHIT results of Group 1 patients before and after surgery were compared, significant differences were detected in the VOR gain averages in certain channels (p<0,05). Again, when the pre- and post-surgical examinations of Group 1 patients were compared, it was determined that the covert saccades that existed before the surgery decreased in the post-surgical period and all covert saccades disappeared at the end of the 2nd week (p<0,05). In conclusion; Despite developing medical and surgical treatment methods, OEM is still an important disease of the pediatric age group that affects patients, families and health policies both financially and spiritually. It is an important cause of dizziness, especially in pediatric patients. Therefore, early diagnosis and medical and surgical treatments with appropriate indications are necessary to prevent it from negatively affecting the quality of life of children and families and the educational life of children. Considering the results of our study, it was thought that surgical treatment may have a positive effect on the vestibular system.
