Öğe Karın Ağrısının Nadir Bir Nedeni: Apendiks Mukosel Olgu Sunumu(2018) Pehlivanlı, Faruk; Karaca, Gökhan; Daphan, Çağatay Erden; Eroğlu, Anıl Oğuz; Vural, Sevilay; Coşkun, FigenApendiksin mukoseli, apendiks lümeninin anormal müsinöz sekresyonu sebebiyle dilatasyonu ile karakterize nadir görülen klinik bir durumdur. İnflamatuar bir duruma veya tümör kaynaklı olabilir. Mukosel, mukozal hiperplazi, müsinöz kistadenom veya müsinöz kistadenokarsinom sonucunda oluşabilir. Apendiks müsinöz kistadenomu en sık görülen şekli olup çok farklı klinik durumlarla kendini belli eder. Apendektomi spesimenlerinin %0.2?0.3’ünde saptanır. Preoperatif tanı konması zordur ve sıklıkla laparotomide saptanır. Rüptür riski, klinik olarak cerrahları zorlamaktadır. Cerrahi planlanmasında tam bir konsensus olmamakla birlikte genellikle apendektomi ya da sağ hemikolektomi ameliyatları uygulanır. Bu yazımızda plastron apendisit tanısıyla takip edilip sağ alt kadran ağrısı ve kitle saptanan apendiks mukosel olgusunu literatür eşliğinde tartıştık.Öğe V. Ulusal Travma ve Acil Cerrahi Kongresi’nde sunulan bildiri özetlerinin öz yazım kılavuzuna uygunluğu değerlendirildi; demografi, metodoloji ve yazım tekniği yönünden incelendi. GEREÇ-YÖNTEM: Kongre kitapçığındaki 451 bildiri değerlendirmeye alındı. Değerlendirilen parametreler; çalışmanın türü ve içeriği, çalışmanın yapıldığı kurumlar, yazar sayısı, başlık kullanımının uyumluluğu, yapısal öz kullanımı, sözcük sayısı, dilin akıcılığı, uygun istatistiksel değerlendirme ve sonuç-öz ilişkisiydi. BULGULAR: Çalışmaların %49’u retrospektif, %29’u olgu sunumuydu. Bildirilerin %33’ü izole organ yaralanması, %26’sı tek sistem yaralanması, %24’ü çoklu sistem yaralanmasına ilişkindi. Çalışmaların %43’ü üniversite, %38’i devlet hastanesi kökenliydi. Çalışmaların %40’ı genel cerrahiyle ilgiliydi, %29’u ise multidisiplinerdi. Çalışmalardaki ortalama yazar sayısı 5,6±1,8 idi; üniversite ve devlet hastaneleri arasında istatistiksel olarak anlamlı farklılık saptandı (p=0,001). Bildirilerin %91’inde yapısal öz vardı, %57’si 100-250, %42’si ise 250-500 sözcük içermekteydi. Çalışmaların %73’ü akıcı olarak değerlendirildi. Tüm bildirilerin %19’unda istatistiksel yöntem uygulandığı belirlendi. Bildirilerin çoğu akıcı idi. İstatistiksel analiz uygulama yönünden üniversite ve devlet hastaneleri arasında anlamlı fark saptandı (p=0,001). Sonuç-öz ilişkisi bildirilerin %71’inde yeterli olarak değerlendirildi. SONUÇ: Bildiri özeti yazma konusunda bazı sorunlarımızın olduğu saptanmıştır. Yapısal öz kullanımı, akıcılık ve yazım kurallarına uyumu artırmaktadır. Özellikle çoklu yaralanlamaları irdeleyen klinik çalışmalar ve deneysel çalışmalar özendirilmeli, bu yönde araştırmalar teşvik edilmelidir.Öğe Kırıkkale Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalında 2013-2017 Yılları Arasında Rastlantısal Safra Kesesi Kanseri Sıklığı(2017) Karaca, GökhanAMAÇ: Safra kesesi tümörleri sıklıkla yaşlılarda görülen agresif seyirli tümörlerdir. Genellikle ilerlemiş halde tanı konabilen safra kesesi tümörlerine erken dönemde kolesistektomi yapılmaksızın tanı konulması oldukça zordur. Tüm gastrointestinal sistem maligniteleri içinde 5. Sıklıkta görülen safra kesesi tümörlerinin tüm evrelerdeki safra kesesi tümörleri göz önüne alındığında tanı sonrası 5 yıllık yaşam oranı %5 olarak görülmektedir. GEREÇ VE YÖNTEMLER: Bu çalışmada Kırıkkale Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalı'nda 2013 -2017 tarihleri arasında taşlı veya taşsız akut ve kronik kolesistit tanısıyla ameliyat edilen 867 hastanın histopatolojik sonuçları incelenmiştir. BULGULAR: Bu dönemde 867 hasta içinde 7 malignite olgusu saptanmış olup 6 tanesi adenokarsinom, 1 tanesi nöroendokrin tümör tanısı almıştır. Serimizde malignite oranı % 0,8 olarak bulunmuştur.SONUÇ: Maligniteden ve bilier komplikasyonlardan kaçınmak için safra kesesi taşlarına erken ameliyat önerilebileceğini düşünmekteyiz.Öğe The Effects of Thymoquinone (Kalonji) on Abdominal Adhesion in Experimental Abdominal Adhesive Model(SPRINGER INDIA, 2020) Ozden, Huseyin; Saygun, Oral; Daphan, Cagatay E.; Aydinuraz, Kuzey; Aydin, Oktay; Tanrikulu, Fatma Benli; Dom, SedatThe aim of this study was to investigate the effects of thymoquinone on abdominal adhesion development in an experimental abdominal adhesion model. Forty-five female Wistar albino rats weighing 260-280 g were used in the study. The rats were randomized into 3 even groups. Control, serum physiological (SP), and thymoquinone (TQ) groups were formed. In the experimental abdominal adhesion model, caecum serosa was brushed until the petechial hemorrhages were seen. No additional procedures were performed in the control group except surgery protocol. SP was injected over the caecum in the SP group. In the thymoquinone group, the abdomen was closed after the application of prepared thymoquinone solution on the caecum surface. On the 21st day, tissue samples from sacrificed rats were examined macroscopically and microscopically, and statistically evaluated. There was a statistically significant difference between the control group and the thymoquinone group in the microscopic evaluation (p = 0.006). However, there was no statistically significant difference between SP group and control group, and between thymoquinone group and SP group. Macroscopic evaluation revealed a statistically significant difference between the thymoquinone group and the control group, and between the thymoquinone group and the SP group (p = 0.009,p = 0.027, respectively). In the microscopic and macroscopic evaluation, it was observed that thymoquinone had a decreasing effect on postoperative adhesions. We think that this effect of thymoquinone is due to its anti-inflammatory and antioxidant properties. However, the available data are not sufficient for this effect to be used in medical treatment. Further studies are needed in the future.Öğe Factors Affecting Mortality in Fournier Gangrene: A Single Center Experience(Mary Ann Liebert, Inc, 2019) Pehlivanli, Faruk; Aydin, OktayBackground: The objective of this study was to determine the clinical and laboratory features affecting mortality in Fournier gangrene. Patients and Methods: This retrospective case control study was designed to evaluate patients treated for Fournier gangrene in our center between 2010 and 2018. Those patients were divided into two groups: discharged patients (group 1) and deceased patients (group 2). Comparisons were made regarding clinical and demographic features; leukocyte, neutrophil and lymphocyte count results; neutrophil to lymphocyte ratio (NLR); Fournier's Gangrene Severity Index (FGSI) scores; number of debridements; complications; and mortality rates. Results: Twenty-three patients (19 males, 4 females) were evaluated; mean age was 65.9116.34 years. The most common cause of the disease and comorbidity were perianal abscess (n=14; 60.9%) and type 1 diabetes mellitus (n=11; 47.8%), respectively. Escherichia coli was the pathogen identified most often (n=17; 73.9%). The total mortality rate was 21.7% (n=5). Neutrophil to lymphocyte ratio, FGSI, number of debridements, and complication rates were higher in group 2 (p<0.05). There was a substantial difference between the groups regarding perianal abscess in group 1 and rectum cancer in group 2 (p<0.05). Conclusion: In conclusion, it was believed that the mortality rate could be predicted by combining the NLR value with the FGSI score.Öğe Ruptured cystic teratoma associated with mucinous cystadenoma in a pregnant woman(Wolters Kluwer Medknow Publications, 2019) Aydin, O.; Pehlivanli, F.; Karaca, G.; Aydin, G.; Sayan, C. D.; Atasoy, P.; Daphan, C. E.Mature cystic teratoma is the most common ovarian neoplasm. However, mucinous cystadenoma with teratoma has been very rarely reported in literature. This case report, which is very rare, describes a clinical entity not previously reported in literature. A 34-year-old pregnant woman presented in the 23rd gestational week with severe right lower quadrant pain. She was diagnosed with acute abdomen and was then treated surgically. During the surgical intervention, a spontaneously ruptured mass was detected in the right ovary. This was reported histopathologically as a mature cystic teratoma in collision with mucinous cystadenoma. To the best of our knowledge, this case report is the first to have identified a ruptured mature cystic teratoma in collision with mucinous cystadenoma in a pregnant woman.Öğe Is the Platelet to Lymphocyte Ratio a Potential Biomarker for Predicting Mortality in Peptic Ulcer Perforation?(Mary Ann Liebert, Inc, 2019) Aydin, Oktay; Pehlivanli, FarukBackground: Despite surgical treatment of peptic ulcer perforation (PUP), the high rates of morbidity and mortality have motivated researchers to search for new laboratory markers to predict morbidity and mortality. The aim of this study was to investigate the relation between pre-operative laboratory values and demographic factors and post-operative mortality in patients undergoing surgery for PUP. Patients and Methods: A retrospective study was made of the clinical findings and laboratory data of patients operated on for a diagnosis of PUP in the general surgery clinic between 2014 and 2018. The patients were separated into two groups according to survival (PUP-S) or mortality (PUP-M) and the differences between the groups were evaluated. Result: In the analysis of the patient data, age (p = 0.014), female gender (p = 0.005), length of stay in hospital (p = 0.009), platelet to lymphocyte ratio (PLR) (p = 0.09), and neutrophil to lymphocyte ratio (NLR) (p = 0.010) values were determined to be high and lymphocyte count was low (p = 0.046) in the PUP-M group. A positive correlation was determined between mortality and age, length of stay in hospital, PLR, and NLR (p < 0.05). A substantial negative correlation was determined between mortality and gender and lymphocyte count (p < 0.05). As a result of the receiver operating characteristic (ROC) curve analysis, it was determined that a PLR value <322.22 (p = 0.009) and lymphocyte count <0.67 x 10(3) microliter (p = 0.035) could have diagnostic value in the prediction of the possibility of mortality in patients operated on because of PUP. Conclusion: This study results suggested that PLR, NLR, and lymphocyte count values could be used as new biomarkers to identify the mortality risk in patients operated on for peptic ulcer perforation.Öğe Significance of Intra-abdominal Free Fluid Detected in Ultrasonography in the Clinical Assessment and Outcomes of Adult Patients Presenting to the Emergency Department Due to Abdominal Pain(Cureus Inc, 2019) Erkek, Aylin; Aydin, Yasemin Yilmaz; Ciftci, Handan; Ramadan, Hayri; Temiz, Kerim; Aydinuraz, Kuzey; Coskun, FigenObjective The aim of the study was to evaluate the diagnostic process and clinical course in adult patients who presented to the emergency department (ED) with acute abdominal pain (AAP) and were found to have intra-abdominal free fluid (FF) on ultrasonography (USG). Methods This prospective observational study was conducted in a training and research hospital adult emergency department between March 15, 2013, and April 15, 2013. The study included 252 patients aged above 18 years, who were admitted to the emergency room complaining of nontraumatic acute abdominal pain and provided consent for the study. Results The most common diagnoses were acute, nonspecific abdominal pain (37.3%), acute appendicitis (19%), and urinary tract pathology (15.9%). Intra-abdominal free fluid was detected with ultrasonography in 42.5% of patients. Patients with intra-abdominal free fluid were younger than the other patients. The emergency department length of stay was longer in patients with intra-abdominal free fluid (p=0.011). Of the 252 patients enrolled in the study, 32.9% were admitted to the hospital, 21.4% of whom underwent surgery and 11.5% received medical therapy. Most of the patients (64.5%) who were discharged home had no intraabdominal free fluid in the ultrasonography (p<0.001). Conclusion The presence of intra-abdominal free fluid alone did not guide the clinical decision regarding the diagnostic evaluation of adult patients that presented to the emergency department complaining of non-traumatic acute abdominal pain.Öğe Role of Platelet to Lymphocyte Ratio as a Biomedical Marker for the Pre-Operative Diagnosis of Acute Appendicitis(Mary Ann Liebert, Inc, 2019) Pehlivanli, Faruk; Aydin, OktayBackground: Recently, the possible use of laboratory parameters such as full blood count, neutrophil count, neutrophil to lymphocyte ratio (NLR), thrombocyte count (PLT), and mean thrombocyte volume (MPV) have been investigated in diagnosis of acute appendicitis and the prediction of complications. The aim of this study was to investigate the utility of the platelet to lymphocyte ratio (PLR) value as a biomedical marker in the prediction of acute appendicitis and perforated appendix. Patients and Methods: The data were retrieved from the hospital records related to age, gender, length of stay in hospital, MPV, leukocyte, neutrophil, lymphocyte, and thrombocyte counts measured in the peripheral venous blood sample on presentation, NLR and PLR values, and the pathology results. The patients were grouped according to the pathology results as follows: group 1 (normal appendix, n = 86); group 2 (acute appendicitis, n = 458); group 3 (perforated appendicitis, n = 14). Results: The study included a total of 558 patients (308 males; 250 females) with a mean age of 34.24 +/- 14.27 years. The mean length of stay in hospital was 2.12 days. The leukocyte and neutrophil count values of group 1 were lower than group 2 and group 3 values (p < 0.001). The lymphocyte count values of group 1 were higher than those of the other groups (p < 0.001). The NLR and PLR values of group 1 were lower than group 2 and group 3 values (p < 0.001). The length of stay in hospital was shorter in group 1 than in group 2 and group 3 (p = 0.42). The receiver operating characteristic (ROC) curve test results showed that PLR and NLR values were sensitive and specific to differentiate normal appendix, acute appendicitis, and perforated appendicitis. Conclusion: The results of this study demonstrated that PLR value, such as NLR, could be evaluated as a new biomarker that could be valuable in the differentiation of normal appendix from acute appendicitis and in the differentiation of acute appendicitis from perforated appendicitis.Öğe Suprapubic Midline Extraperitoneal Approach for Widespread Retroperitoneal Abscess Originating from Anorectal Abscess(Southeastern Surgical Congress, 2018) Aydin, Oktay; Pehlivanli, Faruk; Karaca, Gokhan; Aydin, Gulcin; Ozler, Ismail; Daphan, CagatayÖğe The effect of mesenchymal stem cell use on intra-abdominal adhesions in a rat model(Korean Surgical Society, 2018) Karaca, Gokhan; Pehlivanli, Faruk; Aydin, Oktay; Altunkaya, Canan; Uzun, Hafize; Niyaz, Mehmet; Bulut, HuriPurpose: Intra-abdominal adhesions (IAA) are among the most frequently seen pathologies in general surgery practice with an increased morbidity and mortality. In the present study, we investigated the effect of locally applied mesenchymal stem cells (MSCs) on IAA. Methods: Twenty-four Wistar Albino rats were used in the study. The rats were divided into three groups including: Sham, control, and MSCs group. On day 0, cecum was reached under anesthesia in all groups, except the Sham group. Scraping with a sponge was performed until petechial bleeding occurred. The control group received no treatment. In the stem cell group, MSCs were applied topically immediately after surgery on adhesions. The rats were sacrificed on day 10 and colon tissues and blood samples were collected for macroscopic, histopathological, and biochemical analysis. Results: In our study, E-selectin, P-selectin, TNF-alpha and IL-1 levels were statistically significantly lower in the MSC group than the control group, while the sham group has the lowest levels. In both the macroscopic and histopathological analyses (ZUhlke's scale), the least amount of adhesion was observed in the Sham group. In addition, although there was less adhesion in the MSC group than the control group, the difference did not reach statistical significance. Conclusion: Topical MSC application immediately after surgery suppresses the inflammatory process. However it was found to be ineffective in histopathological and macroscopic examinations performed on the 10th day.Öğe What is the protective effect of metformin on rat ovary against ischemia-reperfusion injury?(Wiley, 2018) Sayan, Cemile Dayangan; Karaca, Gokhan; Ozkan, Zehra Sema; Tulmac, Ozlem B.; Isik, Asli Ceylan; Devrim, Tuba; Yeral, IlkinAimThe aim of this study was to investigate the protective effect of metformin on the rat ovary against ischemia-reperfusion injury. MethodsThirty-seven female Wistar albino rats were used in the study. The rats were divided into five groups, as follows: sham operation group (group 1); torsion group (group 2); torsion/detorsion+saline group (group 3); torsion/detorsion+low-dose metformin group (group 4); and torsion/detorsion+high-dose metformin group (group 5). The right ovary from each rat was evaluated histologically using hematoxylin-eosin staining, and the left ovaries were evaluated for tissue levels of the reduced-glutathione-to-oxidized-glutathione ratio, malondialdehyde (MDA), and caspase-3 activation. ResultsThe highest damage score was observed in group 3, and the lowest score was observed in group 1. The tissue caspase-3 activity levels of groups 2, 3, and 4 were significantly higher than those of group 1. The difference between group 1 and group 5 in terms of tissue caspase-3 activity was not significant (P=0.4). The reduced-glutathione-to-oxidized-glutathione ratio of group 1 was significantly higher than the ratios found in groups 2, 3, and 4. The tissue MDA level of group 1 was significantly lower than the levels found in groups 2, 3, 4, and 5. The tissue MDA level of group 5 was significantly lower than the levels in groups 3 and 4. ConclusionFrom both histopathological and biochemical analyses, the results of the study demonstrated that metformin has beneficial effects when it comes to attenuating ovarian ischemia-reperfusion injury.Öğe Platelet-Rich Plasma May Offer a New Hope in Suppressed Wound Healing When Compared to Mesenchymal Stem Cells(Mdpi, 2018) Aydin, Oktay; Karaca, Gokhan; Pehlivanli, Faruk; Altunkaya, Canan; Uzun, Hafize; Ozden, Huseyin; Guler, OsmanBackground: The present study investigated the effectiveness of platelet-rich plasma (PRP) and mesenchymal stem cells (MSCs) in wound healing suppressed by corticosteroid in rats. Methods: Forty rats were separated into four groups. To disrupt the wound-healing processes, intraperitoneal single dose 10 mg/kg methylprednisolone was administered to all rats with the exception of Sham-S group. Then, full-thickness incision was performed to the abdominal skin of all animals, and PRP or MSCs were applied to the incision line except the Sham-S and Sham-M group animals. Ten days later, all animals were sacrificed to investigate: tissue collagenization, inflammation, and re-epithelialization grades histopathologically; and tissue hydroxyproline (HP), interleukin-1 beta (IL-1 beta), tumor necrosis factor-alpha levels biochemically. Results: Collagenization (p = 0.003) and inflammation grade (p = 0.002) values were higher in PR group. Tissue HP level value was found to be high in MC group (p < 0.001). Tissue IL-1 beta level value of Sham-M group was lower than those of other groups (p < 0.001). Conclusions: This preliminary study revealed that PRP could improve the histopathological grades in wound healing which was suppressed by corticosteroid in rats, while MSCs could show their therapeutic effects via biochemical route. These positive effects were more salient in PR group.Öğe Healing of Ischemic Colon Anastomosis in Rats Could Be Provided by Administering Dexpanthenol or Coenzyme Q10(Mdpi, 2018) Pehlivanli, Faruk; Aydin, Oktay; Karaca, Gokhan; Aydin, Gulcin; Devrim, Tuba; Bulut, Huri; Daphan, Cagatay ErdenBackground: In this study, the effectiveness of dexpanthenol and coenzyme Q10 (CoQ10) on the healing of ischemic colon anastomosis was investigated. Methods: Forty eight maleWistar Albino rats were divided into four equal groups (Sham-S, Sham-I, DXP, Q10). Following full layer colon resection, single layer colon anastomosis, without creating ischemia, was performed on the Sham-S group. The same experimental model was performed on remaining groups after ischemia was created. Intraperitoneal dexpanthenol and CoQ10 was administered to the DXP and Q10 groups once a day for three days. Ten days later, all colon anastomoses were investigated histopathologically and biochemically, as well as their burst pressure values, in all sacrificed rats. Results: The highest burst pressure value was observed in the Sham-S group, decreasing from high to low in the DXP, Q10, and Sham-I groups, respectively (p = 0.008). Furthermore, tissue hydroxyproline (p = 0.001) level values were significantly different among the groups. Additionally, histopathological analysis revealed a significant difference among groups regarding reepithelization (p = 0.027) and polymorphonuclear leukocyte density (p = 0.022). Conclusions: This preliminary study has shown that ischemia-reperfusion injury may impair the healing of colon anastomosis and it has been concluded that dexpanthenol and CoQ10 may have positive effects on the healing of ischemic colon anastomosis in rat, although re-epithelization may be adversely affected using CoQ10.Öğe The Efficacy of Ultrasound-Guided Paravertebral Block in Laparoscopic Cholecystectomy(Mdpi, 2018) Aydin, Gulcin; Aydin, OktayBackground and objectives: Despite its wide use in thoracic procedures, to date, few studies have assessed the effectiveness of paravertebral block (PVB) in laparoscopic cholecystectomy (LC) in an adult population. In these studies, PVB was performed bilaterally using nerve stimulator guidance. To the best of our knowledge, the effectiveness of unilateral preoperative and postoperative ultrasound-guided PVB has not been evaluated in patients undergoing elective LC. The aim of this study was to evaluate the efficacy of single-dose unilateral paravertebral block (PVB) in patients undergoing laparoscopic cholecystectomy (LC) under general anesthesia. Materials and Methods: Patients undergoing LC were randomly separated into control, preoperative block, and postoperative block groups. PVB was performed unilaterally using bupivacaine under ultrasound guidance. Postoperative pain within the first 24 h, side effects, intraoperative opioid and postoperative analgesic requirements were noted. Evaluation was made of a total of 90 patients (25 males, 65 females) with a mean age of 45.78 +/- 14.0 years (range, 19-74 years). Results: Opioid and additional analgesic needs and nausea/vomiting rates were significantly reduced in the preoperative block group compared to the other groups (p < 0.05). Visual Analog Scale (VAS) scores were significantly lower in the preoperative and postoperative block groups compared to the control group (p < 0.05 for all). When the VAS scores were compared between the preoperative and postoperative block groups, a significant difference in favor of the preoperative group was observed in terms of the zero minute-, 1st and 2nd h assessments (p < 0.05 for all). Conclusions: Ultrasound-guided PVB is a useful and safe approach for pain management during and after LC. Preoperative block can also reduce the rate of requirement for intraoperative opioid and postoperative analgesia.Öğe The effect of thymoquinone coating on adhesive properties of polypropylene mesh(Bmc, 2017) Aydin, Oktay; Aydinuraz, Kuzey; Agalar, Fatih; Sahiner, I. Tayfun; Agalar, Canan; Bayram, Cem; Atasoy, PinarBackground: An incisional hernia is a common complication following abdominal surgery. Polypropylene mesh is frequently used in the repair of such defects and has nearly become the standard surgical treatment modality. Though they are very effective in reducing recurrence, mesh materials exhibit a strong stimulating effect for intraabdominal adhesion. The thymoquinone (TQ) extracted from Nigella sativa seeds has potential medical properties. TQ has antiinflammatory, antioxidant and antibacterial properties. The aim of this study is to coat polypropylene mesh with TQ in order to investigate the effect of surface modification on intraabdominal adhesions. Methods: TQ-coated polypropylene mesh material was tested for cytotoxicity, contact angle, surface spectroscopy, TQ content, sterility, and electron microscopic surface properties. An experimental incisional hernia model was created in study groups, each consisting of 12 female Wistar rats. The defect was closed with uncoated mesh in control group, with polylactic acid (PLA)coated mesh and PLA-TQ coated mesh in study groups. Adhesion scores and histopathologic properties were evaluated after sacrifice on postoperative 21th day. Results: Granuloma formation, lymphocyte and polymorphonuclear leukocyte infiltration, histiocyte fibroblast and giant cell formation, capillary infiltration, collagen content were significantly reduced in the PLA-TQ coated mesh group (p < 0.05). Though not statistically significant, likely due to the limited number of study animals, adhesion formation was also reduced in the PLA-TQ coated mesh group (p: 0.067). Conclusion: TQ coated mesh is shown to reduce adhesion formation and TQ is a promising coating material for mesh surface modification.Öğe Effectiveness of thymoquinone, zeolite, and platelet-rich plasma in model of corrosive oesophagitis induced in rats(Korean Surgical Society, 2017) Karaca, Gokhan; Aydin, Oktay; Pehlivanli, Faruk; Altunkaya, Canan; Uzun, Hafize; Guler, OsmanPurpose: The effectiveness of platelet-rich plasma (PRP), thymoquinone, and zeolite-in corrosive esophageal burns was investigated in a rat model. Methods: Four groups were comprised as containing 10 rats in each group. For group I, oesophagitis was induced and no other procedure was performed (control group). For group II, oesophagitis was induced and thymoquinone was administered for 1 week via oral gavage once a day (thymoquinone group). For group III, oesophagitis was induced for 1 week via oral gavage once a day (PRP group). For group IV, oesophagitis was induced and zeolite was administered for 1 week via oral gavage once a day (zeolite group). On the 10th day, the rats were sacrificed under anaesthesia and venous blood sampling was performed from the vena portae. The oesophaguses were totally excised.-Biochemically, interleukin. (IL)-1B, IL-6, TNF-alpha, and MCP-1 were examined from venous blood. Inflammation score was evaluated histopathologically in oesophageal tissue that was collected. Results: There was a statistically significant difference among groups in terms of IL-1, IL-6, MCP levels, compared to the control group; median IL-1, IL-6, MCP levels of thymoquinone, PRP, and zeolite groups were statistically significantly' lower. There was a statistically significant difference among groups in terms of inflammation scores, compared to group I; median inflammation scores of groups II, III and IV were statistically significantly lower thymoquinone. Conclusion: PRP, and zeolite exhibited positive effect on recovery in oesophagitis by reducing inflammation in the involved segment.Öğe The Effects of Simvastatin on Ischemia Reperfusion Injury in an Experimental Colon Anastomosis Model(Springer India, 2017) Akarsu, Mahmut; Saygun, Oral; Aydinuraz, Kuzey; Aydin, Oktay; Daphan, Cagatay Erden; Tanrikulu, Fatma Benli; Comu, Faruk MetinAnastomotic leakage is more frequently reported in colonic anastomoses. Ischemia reperfusion injury is one of the main reasons for anastomotic leakage. Simvastatin is known to prevent tissue damage induced by free oxygen radicals after ischemia reperfusion injury. The effect of simvastatin on colonic anastomosis impaired by ischemia reperfusion injury is investigated. Single layer, end-to-end colocolic anastomosis after 0.5-cm colon resection was performed in Wistar Albino rats. In Group 1 (control) (n= 10), colonic anastomosis without I-R was performed. In Group 2 (n = 10), the superior mesenteric artery was clamped for 10 min followed by 60 min of reperfusion after which resection anastomosis was performed. In Group 3 (n= 10), 10 mg/kg simvastatin was given by gavage for 7 days after I-R and resection anastomosis. In Group 4 (n = 10), the rats received 10 mg/kg simvastatin by gavage 7 days before and 7 days after ischemia reperfusion and surgery. All of the rats were sacrificed 8 days after surgery. Anastomotic bursting pressure and tissue hydroxyproline levels were measured. Postoperative administration of simvastatin restored the anastomotic bursting pressure and hydroxyproline levels to that of control group thus overcoming the effect of ischemia reperfusion injury. Simvastatin administered postoperatively in an experimental model of colonic resection anastomosis impaired by ischemia reperfusion injury increased anastomotic bursting pressures and tissue hydroxyproline levels. Further experimental and clinical studies will show whether administration of simvastatin will increase reliability of the anastomosis and decrease postoperative morbidity and mortality in colonic anastomosis after ischemia reperfusion injury.Öğe Effect of hypothermia on apoptosis in traumatic brain injury and hemorrhagic shock model(Elsevier Sci Ltd, 2017) Eroglua, Oguz; Deniz, Turgut; Kisa, Ucler; Atasoy, Pinar; Aydinuraz, KuzeyIntroduction: The neuroprotective mechanisms of therapeutic hypothermia against trauma-related injury have not been fully understood yet. In this study, we aimed to investigate the effects of therapeutic hypothermia on biochemical and histopathological markers of apoptosis using Traumatic brain injury (TBI) and hemorrhagic shock (HS) model. Methods: A total of 50 male albino-wistar rats were divided into five groups: Group isolated TBI, Group NT (HT + HS + normothermia), Group MH (HT + HS + mild hypothermia), Group MoH (HT + HS + moderate hypothermia) and Group C (control). Neurological deficit scores were assessed at baseline and at 24 h. The rats were, then, sacrificed to collect serum and brain tissue samples. Levels of Caspase-3,6,8, proteoglycan-4 (PG-4), malondialdehyde (MDA), and nitric oxide (NO) were measured in serum and brain tissue samples. Histopathological examination was performed in brain tissue. Results: There were significant differences in the serum levels of Caspase-3 between Group NT and Group C (p = 0.018). The serum levels of Caspase-6 in Group NT (0.70 +/- 0.58) were lower than Group MH (1.39 +/- 0.28), although the difference was not statistically significant (p = 0.068). There were significant differences in the brain tissue samples for Caspase-3 levels between Group NT and Group C (p = 0.049). A significant difference in the Caspase-8 brain tissue levels was also observed between Group NT and Group C (p = 0.022). Group NT had significantly higher scores of all the pathological variables (for edema p < 0.017; for gliosis p < 0.001; for congestion p < 0.003, for hemorrhage p < 0.011) than Group C. Conclusion: Our study results suggest that hypothermia may exert its neuroprotective effects by reducing markers of apoptotic pathway, particularly Caspase-3 on TBI and HS. (c) 2017 Elsevier Ltd. Recep; Yildirim, YesimEctopic liver is a rare developmental abnormality. It is often asymptomatic and could be determined during the surgery. Although it be detected in different areas of the body either below or above of the diaphragm, ectopic liver is usually found on the wall of the gallbladder. The importance of the ectopic liver came from the elevated risk of development of hepatocellulary carcinoma from ectopic tissue. Ectopic liver tissue could also mimic malign masses in radiographic studies. Ultrasound-guided percutaneous biopsies could be helpful for preoperative diagnosis. Recently, widespread usage of laparoscopic techniques caused an increase on the description of ectopic liver tissues located on the gallbladder, Due to the potential risk of developing malignancy the resection of the mass should be the preferred approach for an incidentally or intraoperatively diagnosed ectopic liver tissue.
